摘　　要：目的观察亲属活体角膜缘移植治疗严重角膜缘干细胞缺乏的临床效果。方法回顾性自身对照研究。纳入同济大学附属同济医院眼科2015年1月至2017年3月因热及化学烧伤造成的严重角膜缘干细胞缺乏13例(17眼),获取适量其直系血亲或亲兄弟姐妹的角膜缘组织移植至受眼,术后随访22~26(24.12±1.31)个月。结果17眼中14眼(82.35%)手术成功。末次随访视力(logMAR,BCVA)为0.7(0.6,1.1),优于术前视力1.8(1.6,2.0)(Z=-3.636,P<0.001),末次随访眼表评分为3(2,4)分,优于术前6(5.5,11.5)分(Z=-3.638,P<0.001)。供眼及受眼均未出现严重的眼部并发症。结论亲属活体角膜缘移植治疗严重角膜缘干细胞缺乏安全、有效。Objective To observe the clinical efficacy of lineal relative limbal allograft for severe limbal stem cell deficiency.Methods This was a retrospective and self-control study.Seventeen eyes of 13 patients with severe limbal stem cell deficiency due to thermal and chemical burns from Jan.2015 to Mar.2017 in the Tongji Hospital Affiliated to Tongji University were consecutively recruited.Appropriate amount of limbal tissue from their lineal relative or siblings were harvested and transplanted to the recipients.Postoperative follow-up was required.The mean follow-up time was 22-26(24.12±1.31)months.Results Fourteen eyes(82.35%)achieved surgical success.The final follow-up visual acuity(logMAR,BCVA)was 0.7(0.6,1.1),which was better than the 1.8(1.6,2.0)before operation(Z=-3.636,P<0.001).The total ocular surface grading score(OSGS)was 3(2,4)scores in the final follow-up time,which was better than the 6(5.5,11.5)scores preoperatively(Z=-3.638,P<0.001).No serious ocular complication occurred in donors and recipients.Conclusion Lineal relative limbal allograft is safe and effective for severe limbal stem cell deficiency.
