角膜胶原交联术后角膜光密度变化的临床研究

摘　　要：目的观察圆锥角膜患者快速角膜胶原交联术后角膜光密度的变化。方法前瞻性研究。收集郑州市第二人民医院2018年1至12月进展期圆锥角膜67例(89眼)纳入本研究。患者均接受快速角膜胶原交联手术,以Pentacam眼前段分析系统观察术后角膜光密度变化。结果交联术后所有层次及区域的角膜光密度均立即升高。术后12个月,角膜中心区直径2.0 mm内及2.1~6.0 mm的角膜前层光密度高于术前(t=2.479,5.703;均P<0.05)。术后12个月角膜中心区直径2.1~6.0 mm的角膜前层光密度与角膜前表面最大曲率值存在负相关(r=-0.632,P<0.05),与裸眼视力及最佳矫正视力均无相关性(P=0.881,0.647)。结论角膜胶原交联术后角膜光密度增高,特别是角膜浅基质层中央区。角膜光密度的增高与术后角膜形态变化存在一定相关性,与视力无相关性。Objective To observe the changes of corneal optical density after accelerated corneal cross-linking in patients with keratoconus.Methods This was a prospective study.Eighty-nine eyes of 67 patients with progressive keratoconus,who underwent accelerated corneal cross-linking,from Jan.2018 to Dec.2018 in Zhengzhou Second People’s Hospital were included.Corneal optical density was measured with the Scheimpflug-based Pentacam corneal densitometry module before and after accelerated corneal cross-linking.Results The density of the different corneal layers and zones all increased immediately after surgery.The anterior corneal optical density of 0-2.0 mm and 2.1-6.0 mm zones remained elevated at 12 months after surgery(t=2.479,5.703;all P<0.05).There was a significant negative correlation between the corneal optical density of anterior layer in 2.1-6.0 mm zone and the maximum keratometry of anterior corneal surface at 12 months after surgery(r=-0.632,P<0.05).But there was no correlation between anterior corneal optical density,and the uncorrected visual acuity,the best corrected visual acuity(P=0.881,0.647).Conclusion The corneal optical density increases after corneal cross-linking,especially in the anterior stroma of the central zone.There is some correlation between the corneal optical density and the corneal topography,but no correlation with the visual acuity.
