胃康汤治疗Hp感染合并慢性萎缩性胃炎临床疗效

摘　　要：目的研究胃康汤治疗幽门螺杆菌(Hp)感染合并慢性萎缩性胃炎(CAG)的疗效及其与酪氨酸激酶1/信号转导子和转录激活子3(JAK1/STAT3)信号通路的关系。方法将天津市滨海新区海滨人民医院中医科和天津中医药大学附属武清中医医院脾胃病科130例Hp感染合并CAG患者采用随机数字表法均分为胃康汤组和对照组各65例,两组均给予三联抗Hp感染治疗,胃康汤组在此基础上加用胃康汤治疗,两组疗程均为4周,观察治疗效果并检测治疗前和治疗4周时胃黏膜慢性炎症分级、肠上皮化生分级以及JAK1和STAT3蛋白相对表达量。结果胃康汤组和对照组治疗有效率分别为84.62%和66.15%(P<0.05),两组Hp转阴率分别为87.69%和73.85%(P<0.05);胃康汤组治疗4周时胃黏膜慢性炎症分级低于对照组(P<0.05);治疗4周时两组胃黏膜JAK1和STAT3蛋白相对表达量均降低(P<0.05),且胃康汤组JAK1和STAT3低于对照组(P<0.05);随着治疗效果提升,Hp感染合并CAG患者胃黏膜JAK1和STAT3蛋白相对表达量降低,且Hp转阴者胃黏膜JAK1和STAT3蛋白相对表达量低于Hp未转阴者(P<0.05)。结论胃康汤治疗Hp感染合并CAG具有良好效果,同时有利于减轻患者胃黏膜炎症反应,其作用机制可能与抑制JAK1/STAT3信号通路有关。OBJECTIVE To study the efficacy of Weikang decoction in the treatment of Helicobacter pylori(Hp)infection with chronic atrophic gastritis(CAG)and its relationship with Janus kinase 1/signal transducer and activator of transcription 3(JAK1/STAT3)signaling pathway.METHODS Total of 130 patients with Hp infection complicated with CAG visiting the Department of Traditional Chinese Medicine of Haibin People’s Hospital of Binhai New Area and Department of Spleen and Gastric Diseases of Wuqing Hospital of Traditional Chinese Medicine of Tianjin University of Traditional Chinese Medicine were divided into the Weikang decoction group and control group according to the random number table method,with 65 cases in each group.Both groups were given triple anti-Hp infection treatment,and Weikang decoction group was additionally given Weikang decotion on this basis,and both groups were treated for 4 weeks.The treatment effects were observed,and the gastric mucosal chronic inflammation grading,intestinal metaplasia grading and protein relative expressions of JAK1 and STAT3 were detected before and at 4 weeks after the treatment.RESULTS The effective rates of treatment in Weikang decoction group and control group were 84.62%and 66.15%(P<0.05),and the Hp negative conversion rates of the two groups were 87.69%and 73.85%(P<0.05),respectively.The gastric mucosal chronic inflammation grading in the Weikang decoction at 4 weeks after treatment was significantly lower than that in the control group(P<0.05).The protein relative expressions of JAK1 and STAT3 in gastric mucosa between the two groups were decreased at 4 weeks after treatment(P<0.05),while the expression levels of JAK1 and STAT3 in the Weikang decotion group were significantly lower than those in the control group(P<0.05).With the improvement of treatment effects,the protein relative expressions of JAK1 and STAT3 in gastric mucosa were reduced among patients with Hp infection and CAG,and the protein relative expressions of JAK1 and STAT3 in gastric mucosa of patients with H
