免疫组化多种抗体联合检测在肝细胞癌与相关良性结节鉴别诊断中的应用

摘　　要：目的探究免疫组化多种抗体联合检测在肝细胞癌与相关良性结节鉴别诊断的准确性。方法选取2016年7月至2019年6月本院收治的肝癌患者38例,另选取同期就诊的肝良性结节增生患者18例,通过探究Glypican-3、Bcl-2及Ki-67抗体在肝脏良性增生性结节及肝细胞癌中的表达差异,得出正确的病理诊断,以确定肝癌以及良性增生性结节的最佳诊断方式。结果肝细胞癌组织Gpc-3表达阳性率为84.21%,良性增生性结节Gpc-3蛋白表达阳性率为0.00%;Bcl-2蛋白在肝癌组织中的阳性表达率为88.89%,在良性增生性结节发生率为55.55%,Ki-67蛋白在肝癌组织中的阳性表达率为76.32%,在良性增生性结节阳性表达率为33.33%;Glypican-3及Ki-67免疫组化在肝癌组织的诊断中表达阳性率最高。结论采用Glypican-3及Ki-67免疫组化联合检测高分化肝细胞癌及增生性结节,准确性较高,值得推广应用。Objective To investigate the accuracy of the combined detection of multiple antibodies by immunohistochemistry in the differential diagnosis of hepatocellular carcinoma and related benign nodules.Methods 38 patients with liver cancer treated in our hospital from July 2016 to June 2019 were selected,and 18 patients with benign nodular hyperplasia of the liver were selected in the same period.By exploring the difference in the expression of Glypican-3,Bcl-2 and Ki-67 antibodies in liver benign proliferative nodules and hepatocellular carcinoma,a correct pathological diagnosis was obtained.To determine the best way to diagnose liver cancer and benign proliferative nodules.Results The positive rate of Gpc-3 expression in hepatocellular carcinoma tissue was 84.21%,the positive rate of benign proliferative nodules was 0.00%,and the positive expression rate of Bcl-2 protein in liver cancer tissues was 88.89%,which occurred in benign proliferative nodules was 55.55%.The positive expression rate of Ki-67 protein in liver cancer tissues was 76.32%,and the incidence of benign proliferative nodules was 33.33%.The positive rates of Glypican-3 and Ki-67 were the highest in the diagnosis of hepatocellular carcinoma.Conclusion The combination of Glypican-3 and Ki-67 immunohistochemistry in the detection of well differentiated hepatocellular carcinoma and proliferative nodules has high accuracy and it is worthy of popularization and application.
