氩氦刀治疗前列腺小细胞癌4例分析并文献复习

摘　　要：目的探讨氩氦刀治疗前列腺小细胞癌的有效性,提高对该疾病的认知水平。方法回顾性分析航天中心医院泌尿外科2013年至2018年收集的4例经氩氦刀治疗的前列腺小细胞癌患者的诊疗及随访过程,并结合文献进行归纳总结。结果4例患者中,1例体检发现前列腺特异性抗原(PSA)升高,其余3例表现为渐进性排尿困难。所有病例经病理及免疫组化确诊为前列腺小细胞癌。4例患者均行前列腺癌氩氦刀冷冻治疗术,术后1例患者拒绝化疗,其余3例患者术后联合化疗和内分泌治疗。患者术后随访5~36个月,其中未行术后化疗的患者在术后2个月时出现生化复发,其余患者恢复良好。结论前列腺小细胞癌是一种罕见的前列腺恶性肿瘤,临床上多为晚期患者,预后较差,治疗上选择以局部手术和化疗为主的综合治疗。Objective To find out the efficacy of treatment with argon-helium knife for prostatic small cell carcinoma,improve our knowledge of the disease.Methods The diagnosis,treatment and follow-up of 4 cases of prostatic small cell carcinoma with argon-helium knife treatment in our hospital from 2013 to 2018 were retrospectively analyzed,and their conclusions were made based on the literature.Results The 4 patients were admitted.Among them,1 patient just had elevated PSA and 3 patients presented with progressive dysuria.All cases were pathologically and immunohistochemically diagnosed with small cell carcinoma of the prostate.All the 4 patients underwent argon-helium knife prostatectomy.After that,1 patient refused chemotherapy,but all the other 3 patients received postoperative chemotherapy and androgen deprivation therapy.The patients were followed up for 5-36 months,among which the patient who did not receive postoperative chemotherapy had biochemical recurrence 2 months after the surgery,and the remaining patients recovered well.Conclusions As a rare malignancy,most prostatic small cell carcinoma patients are of advanced stage and poor prognosis.A comprehensive therapy including local treatment or chemotherapy is usually needed.
