摘  要:目的分析比较磁共振增强成像(DCE-MRI)和超声造影(CEUS)评价卵巢肿瘤血管的价值。方法术前对85例卵巢肿瘤患者分别进行DCE-MRI成像和CEUS,术后对切除标本进行免疫组织化学检测,采用CD34单克隆抗体标记血管内皮细胞,计算肿瘤微血管密度(MVD),分析对比卵巢肿瘤组织中DCE-MRI参数和CEUS与MVD的相关性。结果在良性和恶性卵巢肿瘤之间,MVD表达存在明显差异(P=0.001);恶性肿瘤中的CEUS参数峰值强度(PI)和曲线下面积(AUC)显著高于良性肿瘤病变中的PI和AUC(P=0.001,P=0.001);在卵巢肿瘤中,PI和AUC均与MVD显著正相关(r=0.637,P=0.001;r=0.511,P=0.001);恶性肿瘤中的DCE-MRI参数K^(trans)、V_(e)显著高于良性肿瘤(P=0.001,P=0.004);在卵巢肿瘤中,K^(trans)、V_(e)均与MVD均显著正相关(r=0.678,P=0.001;r=0.529,P=0.001);对于卵巢恶性肿瘤,在敏感性、特异性、阴性预测值、阳性预测值和诊断率方面,CEUS与CE-MRI无显著差异(P>0.05)。结论DCE-MRI参数与CEUS参数均可以反映卵巢肿瘤组织MVD的表达水平;CEUS在诊断卵巢恶性肿瘤准确性、特异性、敏感性等方面与DCE-MRI相似,有望成为DCE-MRI之外评估卵巢肿瘤良恶性的重要辅助检查手段。Objective To analyze and compare the value of Dyamiccontrast-enhanced Magnetic Resonance Imaging(DCE-MRI)andContrast-enhanced Ultrasonography(CEUS)in evaluating ovarian tumor blood vessels.Methods Before operation,85 patients with ovarian tumors were subjected to contrast-enhanced ultrasound and magnetic resonance imaging.After the operation,the resected specimens were subjected to immunohistochemical detection.CD34 monoclonal antibody was used to label vascular endothelial cells to calculate tumor microvessel density(MVD),to analyze the correlation between ultrasound contrast parameters and magnetic resonance enhancement parameters and MVD in ovarian tumor tissue.Results There was a significant difference in MVD expression between benign and malignant ovarian tumors(P=0.001);the peak intensity(PI)and area under the curve(AUC)of contrast-enhanced ultrasound parameters in malignant tumors were significantly higher than those in benign tumors and AUC(P=0.001,P=0.001);in ovarian tumors,PI and AUC were significantly positively correlated with MVD(r=0.637,P=0.001;r=0.511,P=0.001);enhancement in malignant tumors.The magnetic resonance parameters K^(trans) and V_(e) were significantly higher than those of benign tumors(P=0.001,P=0.004);in ovarian tumors,both K^(trans) and V_(e) were significantly positively correlated with MVD(r=0.678,P=0.001;r=0.529,P=0.001).For ovarian malignant tumors,there was no significant difference between CEUS and DCE-MRI in terms of sensitivity,specificity,negative predictive value,positive predictive value and diagnostic rate(P>0.05).Conclusion Both DCE-MRI parameters and CEUS parameters can reflect the expression level of MVD in ovarian tumor tissues.CEUS is similar to DCE-MRI in terms of the accuracy,specificity,and sensitivity in diagnosing malignant ovarian tumors,and is expected to become an important method for evaluating benign and malignant ovarian tumors in addition to DCE-MRI.
