摘 要:目的:评价患者胆道梗阻程度与脂质过氧化水平的关系。方法:选择胆道梗阻患者140例,未行胆道穿刺引流,年龄40~64岁,性别不限,BMI 18.5~23.9 kg/m^(2),ASA分级Ⅱ或Ⅲ级,参照Child-Pugh分级总胆红素(TBIL)浓度,按照胆道梗阻程度将患者分为4组(n=35):A组TBIL浓度<17μmol/L;B组17μmol/L≤TBIL浓度<34μmol/L组;C组34μmol/L≤TBIL浓度<51μmol/L组;D组TBIL浓度≥51μmol/L组。测定患者血清TBIL、直接胆红素(DBIL)、间接胆红素(IBIL)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆汁酸(TBA)和丙二醛(MDA)浓度。血清MDA浓度与胆道梗阻程度行Spearman相关分析。结果:与A组和B组比较,C组和D组血清DBIL、IBIL、ALT、AST、TBA和MDA浓度增加(P<0.05),A组和B组上述各指标比较差异无统计学意义(P>0.05);与C组比较,D组血清DBIL、IBIL、ALT、AST、TBA和MDA浓度(P<0.05)。血清MDA浓度与胆道梗阻程度呈正相关(r=0.54,P<0.05)。结论:患者胆道梗阻程度可用于反映脂质过氧化水平。Objective To evaluate the relationship between degrees of biliary obstruction and levels of lipid peroxidation in patients.Methods A total of 140 patients of both sexes,with biliary obstruction,without biliary puncture and drainage,aged 40-64 yr,with body mass index of 18.5-23.9 kg/m^(2),of American Society of Anesthesiologists physical statusⅡorⅢ,were selected.The patients with different degrees of biliary obstruction were divided into 4 groups(n=35 each)according to Child-Pugh grade total bilirubin(TBIL)concentrations:group A(TBIL<17μmol/L),group B(17μmol/L≤TBIL<34μmol/L),group C(34μmol/L≤TBIL<51μmol/L)and group D(TBIL≥51μmol/L).The serum TBIL,direct bilirubin(DBIL),indirect bilirubin(IBIL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bile acid(TBA)and malondialdehyde(MDA)concentrations were measured.The correlation between serum MDA concentration and degree of biliary obstruction was tested by Spearman correlation analysis.Results Compared with group A and group B,the serum DBIL,IBIL,ALT,AST,TBA and MDA concentrations were significantly increased in group C and group D(P<0.05),and there was no significant difference in the parameters mentioned above between group A and group B(P>0.05).Compared with group C,the serum DBIL,IBIL,ALT,AST,TBA and MDA concentrations were significantly increased in group D(P<0.05).Serum MDA concentration was positively correlated with degree of biliary obstruction(r=0.54,P<0.05).Conclusion The degree of biliary obstruction can reflect the level of lipid peroxidation in patients.
