摘 要:目的探讨富士能智能分光染色放大内镜(FICE)检查联合小探头超声内镜(MPS)检查对结直肠病变的诊断价值。方法回顾性分析2018年3月至2020年3月在我院接受FICE检查及MPS检查的72例结直肠病变患者的临床资料,病变类型为隆起型、平坦型及浅表凹陷型。记录FICE检查所观察到病变腺管开口分型及黏膜毛细血管分型情况,分析单独FICE检查及FICE检查联合MPS检查判断病变性质的效能。结果大体病变形态方面,观察到病变分型情况包括Ⅰ_(p)、Ⅰ_(sp)、Ⅰ_(s)、Ⅱ_(a)、LST、Ⅱ_(c)。不同大体形态的病变其腺管开口分型及黏膜毛细血管分型存在较大的差别。单独FICE检查判断病变性质的敏感度、特异度及准确度分别为77.3%、82.1%及79.2%,FICE检查联合MPS检查则相应分别为93.2%、89.3%及91.7%。结论与单独FICE检查相比,FICE检查联合MPS检查可提高镜下判断结直肠病变性质的效能。Objectives To investigate the diagnostic accuracy of Fujinon intelligent chromoendoscopy(FICE)with miniprobeultra⁃sonography(MPS)for colorectal lesions.Methods This was a retrospective analysis of 72 patients with colorectal lesions who un⁃derwent FICE with MPS between March 2018 and March 2020.Types of lesions included elevated type,flat type,and shallow depressed type.The pit pattern and Sano’s capillary pattern of lesions under FICE were recorded.The diagnostic accuracy of FICE alone for characterizing lesions was compared to combining FICE and MPS.Results In terms of gross appearance of the lesions,observed types of lesions includedⅠ_(p),Ⅰ_(sp),Ⅰ_(s),Ⅱ_(a),LST,andⅡ_(c).There were great variations in the pit pattern and Sano’s capillary pattern across different gross appearances of lesions.Sensitivity,specificity,and overall accuracy were 77.3%,82.1%,and 79.2%,respectively for FICE alone,compared with 93.2%,89.3%,and 91.7%for combining FICE and MPS.Conclusion Compared with FICE alone,combining FICE and MPS can improve the diagnostic accuracy of characterizing rectal lesions.
