摘　　要：目的探讨血管内皮生长因子(VEGF)、神经元特异性烯醇化酶(NSE)及可溶性B7家族成员H4(sB7-H4)在小儿恶性淋巴瘤中的表达及诊断价值。方法取54例恶性淋巴瘤患儿的淋巴瘤组织和距肿瘤组织3 cm处的正常组织,免疫组化法检测VEGF、NSE及sB7-H4的表达情况。分析不同临床特征恶性淋巴瘤患儿淋巴瘤组织中VEGF、NSE及sB7-H4的表达情况,绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估VEGF、NSE及sB7-H4单独及联合检测对小儿恶性淋巴瘤的诊断价值。结果恶性淋巴瘤组织中VEGF、NSE及sB7-H4阳性表达率均高于正常组织(P﹤0.05)。临床分期为Ⅲ~Ⅳ期恶性淋巴瘤患儿淋巴瘤组织中VEGF、NSE及sB7-H4阳性表达率均高于Ⅰ~Ⅱ期患儿(P﹤0.05)。ROC曲线结果示,VEGF、NSE、sB7-H4单独检测诊断小儿恶性淋巴瘤的AUC分别为0.632、0.731、0.674,VEGF+NSE+sB7-H4联合检测的AUC为0.805,明显较单独检测高。结论VEGF、NSE及sB7-H4在恶性淋巴瘤中呈异常表达,三者联合检测并结合临床特征有助于提高恶性淋巴瘤的诊断效能。Objective To explore the expression and diagnostic value of vascular endothelial growth factor(VEGF),neuron specific enolase(NSE)and soluble soluble B7 family member H4(sB7-H4)in pediatric malignant lymphoma.Method The lymphoma tissues and normal tissues 3 cm away from the tumor tissues from 54 children with malignant lymphoma were collected,and the expressions of VEGF,NSE and sB7-H4 were detected by immunohistochemistry.To analyze the expression of VEGF,NSE and sB7-H4 in lymphoma tissues of children with malignant lymphoma with differ-ent clinical characteristics.Draw the receiver operating characteristic(ROC)curve and calculate area under the curve(AUC)to evaluate the diagnostic value of VEGF,NSE and sB7-H4 alone and in combination in the diagnosis of malig-nant lymphoma in children.Result The positive expression rates of VEGF,NSE and sB7-H4 in malignant lymphoma tis-sue were higher than those in the control group(P<0.05).The positive expression rates of VEGF,NSE and sB7-H4 in lymphoma tissues of children with clinical stage III-IV malignant lymphoma were higher than those of children with stage I-II(P<0.05).The ROC curve results showed that the AUC of VEGF,NSE,and sB7-H4 for the diagnosis of pediat-ric malignant lymphoma were 0.632,0.731,and 0.674,respectively,and the AUC of the combined detection of VEGF+NSE+sB7-H4 was 0.805,which was significantly higher than that of the single detection.Conclusion VEGF,NSE and sB7-H4 show abnormal expression in patients with malignant lymphoma.The combined detection of three levels,com-bined with the clinical characteristics of patients,will help improve the value in the diagnosis of malignant lymphoma.
