血清鳞状细胞癌抗原、胃泌素释放肽前体在肺癌中的表达及对肺癌的诊断价值

摘　　要：目的探讨血清鳞状细胞癌抗原(SCCA)、胃泌素释放肽前体(ProGRP)在肺癌中的表达及对肺癌的诊断价值。方法选取108例肺癌患者(作为肺癌组)、45例肺良性疾病患者(作为肺良性病变组)和45例健康体检者(作为健康组)。化学发光法检测3组受试者血清SCCA、ProGRP水平,比较不同临床特征肺癌患者血清SCCA、ProGRP的表达情况。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估SCCA、ProGRP单独及联合检测对肺癌的诊断价值。结果肺癌组患者血清SCCA、ProGRP水平均高于肺良性病变组和健康组(P﹤0.05),肺良性病变组患者血清SCCA、ProGRP水平均高于健康组(P﹤0.05)。临床分期为Ⅰ~Ⅱ期、中高分化、无淋巴结转移肺癌患者血清SCCA、ProGRP水平和阳性表达率均低于临床分期为Ⅲ~Ⅳ期、低分化和有淋巴结转移的患者(P﹤0.05)。SCCA+ProGRP联合检测诊断肺癌的AUC为0.845,此时的灵敏度和特异度均较高。结论SCCA、ProGRP在肺癌中高表达,其表达情况可能与肺癌患者的分化程度、TNM分期、淋巴结转移情况有关,SCCA、ProGRP联合检测可提高肺癌的诊断效能。Objective To explore the expression of serum squamous cell carcinoma antigen(SCCA)and pro-gastrin releasing peptide(ProGRP)in lung cancer and their diagnostic value for lung cancer.Method Selected 108 lung cancer patients(as the lung cancer group),45 patients with benign lung diseases(as the lung benign lesion group),and 45 healthy subjects(as the healthy group).Chemiluminescence method was used to detect the levels of serum SCCA and ProGRP in the three groups of subjects,and to compare the expression of serum SCCA and ProGRP in patients with lung cancer with different clinical characteristics.Draw the receiver operating characteristic(ROC)curve,calculate area under the curve(AUC),and evaluate the diagnostic value of SCCA and ProGRP alone and combined detection for lung cancer.Result The levels of serum SCCA and ProGRP in the lung cancer group were higher than those in the lung benign lesion group and the healthy group(P<0.05),and the serum SCCA and ProGRP levels in the benign lung disease group were higher than those in the healthy group(P<0.05).The levels of serum SCCA and ProGRP and the positive expression rate of lung cancer patients with clinical stage I-II,moderately well differentiated and no lymph node metastasis were lower than those of patients with clinical stage III-IV,poorly differentiated and lymph node metastasis(P<0.05).The AUC of the combined detection of SCCA+ProGRP in the diagnosis of lung cancer is 0.845,and the sensitivity and specificity are both high at this time.Conclusion SCCA and ProGRP were highly expressed in lung cancer,and their expression may be related to the differentiation of lung cancer patients,TNM staging,and lymph node metastasis.The combined detec-tion of SCCA and ProGRP can improve the diagnostic efficiency of lung cancer.
