摘　　要：目的探讨全科医学科团队在慢病管理中应用血糖仪APP、“糖友会”、微信群等多种方式进行出院后全程管理的效果。方法2019年9月-2020年3月随机选择符合入组标准的我院全科医学科住院糖尿病患者80例,分为观察组40例、对照组40例,分别予以血糖仪APP实时传输血糖数据、组织“糖友会”、微信群内相互交流结合每月一次座谈形式健康讲座和传统电话随访。干预6个月后,比较两组自我管理情况、血糖控制情况及生活质量。结果干预6个月后两组患者BMI、空腹血糖(FBG)、餐后2h血糖(2h PG)、糖化血红蛋白(HbA1c)、患者知识行为问卷、糖尿病患者特异性生存质量特异性量表(DSQL)(P<0.05)。观察组较对照组在自我管理情况、血糖控制情况及生活质量方面均优于对照组。结论全科团队运用新兴智能化管理方式提高了管理效率和患者参与度,效果优于传统方式。Objective Discuss the effect of the general practice team in the management of chronic diseases using the blood glucose meter APP,"Sugar Friends Club",WeChat group and other methods to manage the whole process after discharged.Methods Randomly select 80 patients with diabetes in our department who meet the enrollment criteria,divided into intervention group with 40 cases and control group with 40 cases,real-time transmission of blood glucose data by the blood glucose meter APP,organization of"Sugar Friends Club",mutual exchanges in the WeChat group,combined with monthly health seminars and traditional telephone follow-ups.ResuIts The intervention group was better than the control group in terms of self-management,blood sugar control and quality of life.Conelusion The general practice team has used new intelligent methods to improve management efficiency and patient participation,and the effect is better than traditional methods.
