甘氨双唑钠对诱导化疗后局部晚期鼻咽癌的增敏研究

摘　　要：目的探讨甘氨双唑钠联合顺铂同期放化疗对诱导化疗后局部晚期鼻咽癌近期和远期疗效的影响。方法将202例病理确诊的初治Ⅲ~Ⅳ期局部晚期鼻咽癌患者随机分为研究组和对照组,研究组100例,对照组102例。2组均使用6MV的X线加速器行调强放疗,鼻咽原发灶处方剂量:68~72 Gy,颈部阳性淋巴结处方剂量:66~70 Gy,淋巴引流区预防照射剂量:高危区(CTV1):60 Gy,低危区(CTV2):54~56 Gy,1次/日,5次/周,共治疗6~7周。2组均先行2周期"吉西他滨+铂类"或"多西他赛+铂类"方案诱导化疗后行同期放化疗,放疗同期使用顺铂80 mg/m^(2)单药化疗。研究组在此基础上同期使用甘氨双唑钠800 mg/m^(2),放疗前1 h静脉滴注,每周3次。结果研究组和对照组的肿瘤完全缓解率分别为94.0%和84.3%,差异有统计学意义(P=0.027)。研究组和对照组的5年总生存率分别为84.4%和64.3%,5年无进展生存率为76.5%和60.7%,研究组比对照组的生存率明显提高,差异有统计学意义(P<0.05)。结论甘氨双唑钠可提高诱导化疗联合同期放化疗模式下局部晚期鼻咽癌的近期疗效及远期疗效。Objective To evaluate the short-term and long-term efficacy of metronidazole amino acidum natrium(CMNa)combined with concurrent cisplatin and radiotherapy for locally advanced nasopharyngeal carcinoma after induction chemotherapy.Methods 202 patients with locally advanced nasopharyngeal carcinoma(NPC)confirmed by pathology were randomly divided into the treatment group(100 patients)and the control group(102 patients).All the patients received radiotherapy with 6 MV linear accelerator and concurrent cisplatin at a dose of 80 mg/m^(2)every 3 weeks.In the treatment group,the patients were given sodium glycididazole at 800 mg/m^(2)before radiotherapy,by l h intravenous drip,3 times a week.Results The complete remission rate of the treatment group and the control group were 94%and 84.3%,respectively,and the difference was statistically significant(P=0.027).In the treatment group,5-year overall survival rate was 84.4%,and in the control group,the corresponding survival rate was 64.3%,5-year disease-free survival rates in the 2 groups were 76.5%and 60.7%,respectively.The treatment group were significantly higher than the control group(P<0.05).Conclusion CMNa combined with concurrent radiochemotherapy can improve the short-term and long-term result of locally advanced nasopharyngeal carcinoma patients.
