丹红注射液联合氢氯噻嗪治疗乳腺癌术后患者上肢淋巴水肿的疗效及对生活质量的影响

摘　　要：目的探讨丹红注射液联合氢氯噻嗪治疗乳腺癌术后患者上肢淋巴水肿的疗效及对生活质量的影响。方法选取乳腺癌术后发生上肢淋巴水肿的患者96例,并随机分为2组,各48例。对照组给予氢氯噻嗪治疗,治疗组在对照组基础上给予丹红注射液,对比2组治疗后临床疗效、KPS评分、中医症候积分、上臂周径差值及血清TNF-α、IL-6、CRP水平。结果治疗后,治疗组总有效率较高(P<0.05)。2组治疗前KPS评分无差异(P>0.05),治疗后2组KPS评分显著升高(P<0.05);并且治疗组升高较明显(P<0.05)。2组治疗前中医症候积分无差异(P>0.05);治疗后2组肢体麻木、上肢水肿、疼痛等评分显著降低(P<0.05);并且治疗组降低较明显(P<0.05)。2组治疗前上臂周径差值无差异(P>0.05);治疗后2组上臂周径差值显著降低(P<0.05);并且治疗组降低较多(P<0.05)。2组治疗前血清TNF-α、IL-6、CRP水平无差异(P>0.05);治疗后2组血清TNF-α、IL-6、CRP水平显著降低(P<0.05);并且治疗组降低较多(P<0.05)。结论采用丹红注射液联合氢氯噻嗪治疗乳腺癌术后患者上肢淋巴水肿具有较好的治疗效果。Objective To investigate the efficacy of Danhong injection combined with hydrochlorothiazide in the treatment of upper extremity lymphedema after breast cancer surgery and its effect on quality of life.Methods 96 patients with upper extremity lymphedema after breast cancer surgery in our hospital were selected and randomly divided into 2 groups,48 patients in each group.The control group was given hydrochlorothiazide,and the treatment group was given Danhong injection on the basis of the control group.After treatment,the clinical efficacy,KPS score,TCM syndrome score,upper arm circumference diameter difference,serum TNF-α,IL-6 and CRP levels of the 2 groups were compared.Results After treatment,the total effective rate was higher in the treatment group(P<0.05).There was no difference in KPS score between the 2 groups before treatment(P>0.05).KPS score was significantly increased after treatment in both groups(P<0.05).The increase in the treatment group was more significant(P<0.05).There was no difference in the scores of TCM syndromes before treatment between the 2 groups(P>0.05).After treatment,the scores of limb numbness,upper limb edema and pain in both groups were significantly reduced(P<0.05).In addition,it was significantly reduced in the treatment group(P<0.05).There was no difference in upper arm circumference diameter between the 2 groups before treatment(P>0.05).After treatment,the difference between the 2 groups was significantly reduced(P<0.05).In addition,there was a significant decrease in the treatment group(P<0.05).There was no difference in serum TNF-α,IL-6 and CRP between the two groups before treatment(P>0.05).The serum levels of TNF-α,IL-6 and CRP in the 2 groups were significantly decreased after treatment(P<0.05).In addition,there was a significant decrease in the treatment group(P<0.05).Conclusion Danhong injection combined with hydrochlorothiazide in the treatment of upper extremity lymphedema after breast cancer has a good therapeutic effect.
