宫颈癌腹腔镜下淋巴结切除术治疗的有效性及影响患者预后相关因素分析

摘　　要：目的探讨宫颈癌患者腹腔镜下淋巴结切除术治疗的有效性及影响患者预后的相关因素。方法回顾性分析153例宫颈癌患者的临床资料,分析宫颈癌患者腹腔镜下淋巴结切除术治疗效果及影响患者预后的因素。结果153例患者经腹腔镜下淋巴结切除术治疗,2年后生存130例,死亡23例。死亡组盆腔淋巴结阳性、低分化、肿瘤直径≥4 cm、发生子宫内膜受累、存在宫颈深层浸润、脉管浸润、临床分期ⅡA发生率均高于生存组,差异有统计学意义(P<0.05);多因素Logistic回归分析显示,盆腔淋巴结阳性、低分化、肿瘤直径≥4 cm、子宫内膜受累、宫颈深层浸润、脉管浸润、临床分期ⅡA是影响宫颈癌患者腹腔镜下淋巴结切除术预后的独立危险因素(P<0.05且OR>1)。结论盆腔淋巴结性质、肿瘤分化程度、肿瘤大小、是否累及子宫内膜及脉管与宫颈深层浸润程度、临床分期等均是影响宫颈癌患者腹腔镜下淋巴结切除术预后的主要因素,在临床治疗中需予以重视。Objective To investigate the effectiveness of laparoscopic lymphadenectomy in patients with cervical cancer and the related factors affecting prognosis.Methods The clinical data of 153 patients with cervical cancer treated were analyzed retrospectively,To analyze the effect of laparoscopic lymphadenectomy and the influencing factors of prognosis in cervical cancer patients.Results A total of 153 patients underwent laparoscopic lymphadenectomy,Of 130 cases after 2 years,there were 23 deaths.The incidence of endometrial involvement,deep cervical invasion,vascular invasion,clinical stage II A≥4 in the death group,difference was statistically significant(P<0.05);A multivariate Logistic regression analysis showed,pelvic lymph node positive,poorly differentiated,tumor diameter≥4 cm、endometrial involvement,deep cervical invasion,vascular invasion,clinical stage ii A are independent risk factors for the prognosis of laparoscopic lymphadenectomy in patients with cervical cancer(P<0.05 and OR>1).Conclusion The nature of pelvic lymph nodes,the degree of tumor differentiation,the size of the tumor,whether the endometrium is involved,the degree of deep invasion of the vein and cervix,and the clinical stage are the main factors affecting the prognosis of laparoscopic lymphadenectomy,It should be paid attention to in clinical treatment.
