摘　　要：目的探讨侧脑室脑膜瘤的磁共振(MR)成像影像学特征与其病理的相关性。方法选择侧脑室脑膜瘤患者172例作为研究对象,病理诊断为良性脑膜瘤140例(良性组)和恶性脑膜瘤32例(恶性组)。所有患者都给予MR成像检查,记录影像学特征并进行相关性分析。结果恶性组的MRI征象颅骨破坏、脑膜尾征、坏死囊变、瘤脑界面等占比都高于良性组(P<0.05)。恶性组的弥散加权成像(DWI)ADC值低于良性组(P<0.05),MTT与TTP值高于良性组(P<0.05)。Pearson分析显示恶性侧脑室脑膜瘤与ADC、MTT、TTP、脑膜尾征、坏死囊变、瘤脑界面都存在相关性(P<0.05)。结论MRI可客观、简单、准确传递侧脑室脑膜瘤的生理学与影像学信息,有助于判定侧脑室脑膜瘤的良恶性病理情况,有很好的临床推广应用价值。Objective To investigate the correlation between the imaging features of lateral ventricle meningioma and its pathology.Methods 172 patients with lateral ventricle meningioma were selected as the research objects.Pathologically diagnosed were 140 cases of benign meningiomas(benign group)and 32 cases of malignant meningioma(malignant group).All patients were given MR imaging examination,imaging characteristics were recorded and given correlation analysis.Results The proportions of MRI signs of skull destruction,meningeal tail sign,necrotic cystic degeneration,and tumor-brain interface in the malignant group were higher than those in the benign group(P<0.05).The diffusion-weighted imaging(DWI)ADC value of the malignant group were lower than that of the benign group(P<0.05),and the MTT and TTP values were higher than that of the benign group(P<0.05).Pearson analysis showed that malignant lateral ventricular meningioma were correlated with ADC,MTT,TTP,meningeal tail sign,necrotic cyst degeneration,and tumor-brain interface(P<0.05).Conclusion MRI can objectively,simply and accurately transmit the physiological and imaging information of lateral ventricular meningioma,which is helpful to determine the benign and malignant pathological conditions of lateral ventricular meningioma,and has good clinical application value.
