摘　　要：目的探究大黄甘草汤结合穴位敷贴对非小细胞肺癌(NSCLC)化疗致胃肠道反应的效果。方法选取2018年4月至2020年4月我院收治的NSCLC患者116例。根据治疗方法不同分为观察组(60例)和对照组(56例),2组均进行肿瘤化疗治疗,对照组采用甲磺酸多拉司琼和地塞米松止吐治疗,观察组在对照组基础上采用大黄甘草汤结合穴位敷贴治疗。治疗3个月,观察2组免疫功能、胃肠道反应发生率、生活质量及不良反应。结果与对照组比较,观察组CD4^(+)、CD4^(+)/CD8^(+)、自然杀伤细胞(NK)水平较高(P<0.05)。观察组恶心、呕吐、食欲减退和腹泻总发生率低于对照组(43%与84%)(P<0.05)。与对照组比较,观察组躯体功能、情绪功能、社会功能、角色功能和认知功能得分较高(P<0.05)。与对照组比较,观察组白细胞减少、中性粒细胞减少比例较小(P<0.05);但2组血小板减少、血红蛋白减少比例比较差异无统计学意义(P>0.05)。结论大黄甘草汤结合穴位敷贴可减少NSCLC化疗引起的胃肠道反应和不良反应,有效提高免疫功能,改善生活质量,具有较好的应用前景。Objective To investigate the effect of Dahuang Gancao Recipe plus acupoint application on gastrointestinal reactions induced by chemotherapy for nonsmall cell lung cancer(NSCLC).Methods A total of 116 NSCLC patients with NSCLC admitted to our hospital between April 2018 and April 2020 were enrolled.According to treatment options,they were divided into the study group(n=60)and control group(n=56),all receiving chemotherapy for NSCLC.The control group was given dolasetron mesylate and dexamethasone as antiemetic agents,while the study group received Dahuang Gancao Recipe plus acupoint application,in addition to the care given in the control group.At 3 months of treatment,the immune function,gastrointestinal reaction,quality of life and toxicity were evaluated in the two groups.Results Compared with the control group,the study group had higher levels of CD4^(+),CD4^(+)/CD8^(+),and NK(P<0.05).The total incidence of nausea,vomiting,loss of appetite and diarrhea in the study group was lower than that in the control group(43%vs 84%)(P<0.05).Compared with the control group,the study group had higher scores of physical performance,emotion,social functioning,role functioning and cognitive functioning(P<0.05).Compared with the control group,the study group was less likely to develop leukopenia and neutropenia(P<0.05).However,the proportions of patients with thrombocytopenia and lowered hemoglobin were not significantly different between the two groups(P>0.05).Conclusion Dahuang Gancao Recipe plus acupoint application can reduce the gastrointestinal reactions and toxicity caused by chemotherapy for nonsmall cell lung cancer,effectively improve the immune function and quality of life,and therefore is promising for use in clinical practice.
