胃癌根治性手术前后TM4SF1水平及病理特征变化研究

摘　　要：目的探讨胃癌根治性手术前后TM4SF1(跨膜4超家族成员1)水平及病理特征变化。方法选取天津市宁河区医院2016年6月到2019年6月共收治的80例胃癌患者进行回顾性分析,所有患者均行胃癌根治术,分析胃癌患者根治术前胃癌组织及邻近非胃癌组织中TM4SF1表达,胃癌患者根治术后胃粘膜中TM4SF1水平表达与胃癌根治术前TM4SF1表达水平对比,并对比所有患者胃癌根治术前和手术1年后的临床病理特征。结果在同一切段中,TM4SF1在GC组织中呈阴性表达,在相邻非癌组织中呈阳性表达。在我们的患者队列中,免疫组化分析显示,在GC组织中TM4SF1的表达率(11.25%,9/80)显著低于非癌性胃粘膜组织(72.50%,58/80);TM4SF1在胃底黏膜组织中的表达率(89.65%,52/58)高于心脏(16.67%,1/6)和幽门(16.67%,2/12)黏膜组织。胃底和化生粘膜中TM4SF1的阳性表达率无显著差异(75.00%,3/4);与匹配的非癌组织样本(7.69%,1/13)相比,这些配对样本在根治术前中表现出较低的TM4SF1表达(61.54%,8/13)。这些数据表明胃癌根治术前TM4SF1水平低于胃癌根治术后;为了探讨胃癌根治术对胃癌患者的治疗效果,对比分析了根治性手术前与根治性手术后1年两组患者的病理特征,结果显示,手术前与手术后胃癌患者的肿瘤Borrmann分型、肿瘤pTNM分期、肿瘤pN分期患者不同分期人数对比差异显著。结论TM4SF1的低表达与胃癌的发生发展、肿瘤的进展和侵袭以及胃癌患者的整体生存率差有关,且胃癌根治性手术后患者TM4SF1水平发生改变,患者的病理特征也出现明显改变。TM4SF1是GC的抑癌因子,也是GC患者新的预后标志物。Objective To investigate the changes of tm4 sf1(transmembrane 4 superfamily member 1)level and pathological characteristics of gastric cancer before and after radical operation.Methods a total of 80 patients with gastric cancer admitted to Tianjin ninghe district hospital from June 2016 to June 2019 were retrospectively analyzed.All patients underwent radical gastrectomy.The expression of tm4 sf1 in gastric cancer tissues and adjacent non gastric cancer tissues before radical gastrectomy was analyzed.The expression of tm4 sf1 in gastric mucosa of patients with gastric cancer after radical resection was compared with that before and after radical gastrectomy The clinicopathological features before and 1 year after radical operation were analyzed.Results in the same segment,tm4 sf1 was negative in GC tissues and positive in adjacent noncancerous tissues.In our cohort,immunohistochemical analysis showed that the expression rate of tm4 sf1 in GC tissue(11.25%,9/80)was significantly lower than that in non cancerous gastric mucosa(72.50%,58/80);the expression rate of tm4 sf1 in gastric fundus mucosa(89.65%,52/58)was higher than that in heart(16.67%,1/6)and pyloric mucosa(16.67%,2/12).There was no significant difference in tm4 sf1 expression between gastric fundus and metaplasia mucosa(75.00%,3/4).Compared with matched non cancer tissue samples(7.69%,1/13),these matched samples showed lower expression of tm4 sf1(61.54%,8/13)before radical operation.These data show that the level of tm4 sf1 before radical gastrectomy is lower than that after radical gastrectomy.In order to explore the therapeutic effect of radical gastrectomy on gastric cancer patients,the pathological characteristics of patients before radical operation and 1 year after radical operation were compared and analyzed.The results showed that the Borrmann classification,pTNM stage and PN stage of gastric cancer patients before and after operation were different There was a significant difference in the number of people.Conclusion the low expression of tm4 sf
