摘  要:目的分析脑卒中后抑郁患者交感神经皮肤反应(SSR)潜伏期、波幅与汉密尔顿抑郁量表(HAMD)评分的相关性。方法选取2019年2月至2020年1月巩义市人民医院收治的74例脑卒中患者为脑卒中组,选取同期60例健康体检者作为正常对照组。分析脑卒中组抑郁状态,比较两组HAMD与上下肢神经SSR潜伏期、波幅水平的相关性,并给予对应治疗,比较治疗前后脑卒中组抑郁者上下肢神经SSR潜伏期、波幅水平。结果74例脑卒中患者经HAMD评分评估后,无抑郁占比32.43%,轻度抑郁占比48.65%,中重度抑郁占比18.92%。脑卒中组SSR潜伏期均较正常对照组延长,SSR波幅均较正常对照组下降(P<0.05)。脑卒中组上肢正中神经、下肢胫后神经SSR潜伏期与HAMD评分呈正相关(r=0.602、0.657,P<0.05),上肢正中神经、下肢胫后神经SSR波幅与HAMD评分呈负相关(r=-0.495、-0.611,P<0.05)。治疗后脑卒中组抑郁者上肢正中神经与下肢胫后神经SSR潜伏期均低于治疗前,波幅均高于治疗前(P<0.05)。结论脑卒中患者存在皮肤交感神经兴奋性降低,脑卒中后HAMD评分越高,SSR潜伏期、波幅变化越明显,监测SSR潜伏期、波幅可为脑卒中后抑郁诊断及治疗提供参考,促进脑卒中后抑郁疾病转归。Objective To analyze the correlation between the incubation period and amplitude of sympathetic skin response(SSR)in patients with post-stroke depression and the Hamilton depression scale(HAMD)score.Methods A total of 74 stroke patients from Gongyi People’s Hospital from February 2019 to January 2020 were selected as the stroke group,and 60 healthy patients during the same period were selected as the normal control group.The depression status of the stroke group was analyzed,and the median nerve of the upper limbs and the tibia of the lower limbs were compared between the two groups.SSR latency,amplitude level of posterior nerve and their correlation with HAMD score,and corresponding treatments were given to compare the SSR latency and amplitude level of upper limb median nerve and lower limb posterior tibial nerve in the stroke group before and after treatment.Results After 74 stroke patients were assessed by the HAMD score,32.43%were without depression,mild depression accounted for 48.65%,and moderate to severe depression accounted for 18.92%.The SSR latency of the median nerve of the upper limb and the posterior tibial nerve of the stroke group was longer than that of the normal control group,and the SSR amplitude was lower than that of the normal control group(P<0.05).The SSR latency of the median nerve of the upper limb and the posterior tibial nerve of the lower limbs in the stroke group were positively correlated with the HAMD score(r=0.602,0.657,P<0.05).and the SSR amplitude of the median nerve of the upper limb and the posterior tibial nerve of the lower limb were negatively correlated with the HAMD score(r=-0.495,-0.611,P<0.05).The SSR latency of the median nerve of the upper limb and the posterior tibial nerve of the lower limbs in the stroke group after treatment was lower than before treatment,and the amplitude was higher than before treatment(P<0.05).Conclusion Patients with stroke have reduced skin sympathetic excitability.The higher the HAMD score after stroke,the more obvious the SSR latency an
