摘  要:目的探讨非X线下超细内镜引导自膨式金属肠道支架置入术在晚期结直肠癌引起的恶性肠梗阻中的临床价值。方法应用超细内镜引导置入自膨式金属肠道支架治疗结直肠恶性梗阻26例,其中直肠8例,乙状结肠7例,降结肠9例,回盲部2例。分析其临床疗效。结果26例结直肠癌引起的恶性肠梗阻患者,肠道支架置入成功率100%(26/26),术后初次排气/排便时间(32.15±4.21)h;随访1~6个月(中位随访时间2.5个月),患者均能进食流质或半流质饮食,无死亡病例。结论经超细内镜引导自膨式肠道金属支架置入术治疗晚期结直肠癌恶性梗阻临床价值良好,可提高患者的生存质量。Objective To evaluate the clinical value of non-X-ray ultra-thin endoscopic guided self-expandable metal intestinal stent implantation in malignant intestinal obstruction caused by advanced colorectal cancer.Methods Clinical data of 26 patients diagnosed as advanced colorectal cancer with malignant obstruction and received self-expandable metallic stent therapy by using ultrathin endoscopy in the First Affiliated Hospital,Guangzhou Medical University from September 2016 to February 2019 were retrospectively analyzed in this study.Results All patients(26/26,100%)were successfully placed the self-expandable metallic stent.Obstruction symptom relief and restoration of bowel movement were observed 32.15±4.21 after the procedure.No complications were found in all patients,including bleeding,perforation.Follow up of one to six months(median:2.5 months),all patients could have a good quality of life under liquid diet or semi-liquid diet.Conclusion Self-expandable metallic stent placement by using ultrathin endoscopy in patients with advanced colorectal obstruction appears to be safe and effective.
