摘　　要：目的 研究透明肛管显示器在腹腔镜下直肠癌经肛门结肠肛管吻合术(Parks术)中的应用价值。方法 38例行腹腔镜下直肠癌Parks术患者,依照治疗环节是否应用透明肛管显示器分为对照组(26例)和观察组(12例)。观察组应用透明肛管显示器行腹腔镜下直肠癌Parks术会阴区操作,对照组采用传统方法行腹腔镜下直肠癌Parks术会阴区操作。比较两组不良事件发生率、手术时长、术中出血量、术后复发率、住院时间和住院费用。结果 观察组不良事件发生率25.0%(3/12)低于对照组的84.6%(22/26),差异有统计学意义(P<0.05)。观察组手术时长(211.8±15.6)min短于对照组的(229.8±15.2)min,术中出血量(130.8±10.3)ml少于对照组的(236.0±10.5)ml,差异有统计学意义(P<0.05)。两组术后复发率比较差异无统计学意义(P>0.05)。观察组患者住院时间(25.4±2.1)d短于对照组的(27.4±2.0)d,住院费用(42.931±1.002)千元低于对照组的(43.837±1.008)千元,差异有统计学意义(P<0.05)。结论 透明肛管显示器在腹腔镜下直肠癌Parks术中的应用价值尤其突出,在会阴区肛门操作时使用透明肛管显示器,使手术进一步改良,在手术时间、术后相关指标及患者预后改善上取得了较佳的治疗效果,值得在该类患者的手术治疗环节推广实施。Objective To study the application value of transparent anal tube display in laparoscopic transanal coloanal anastomosis(Parks operation)for rectal cancer.Methods A total of 43 patients who underwent laparoscopic Parks operation for rectal cancer were divided into control group(29 patients)and observation group(14 patients)according to whether a transparent anal tube display was applied during treatment.The control group received laparoscopic Parks operation for rectal cancer by traditional method,and the observation group received laparoscopic Parks operation for rectal cancer by transparent anal tube display.The incidence of adverse events,surgery-related indicators and other postoperative indicators were compared between the two groups.Results The incidence of adverse events 7.14%(1/14)in the observation group was lower than 37.93%(11/29)in the control group,and the difference was statistically significant(P<0.05).The operation time(17.84±2.05)min of the observation group was shorter than(23.23±3.18)min of the control group,and the intraoperative blood loss(151.35±10.26)ml was less than(226.42±10.57)ml of the control group.All the differences were statistically significant(P<0.05).The healing time of surgical incision(2.91±0.30)d of the observation group was shorter than(5.75±0.54)d of the control group,and the postoperative recurrence rate 0 and hospitalization cost(4.45±0.65)thousand yuan were significantly lower than 24.14%and(6.12±0.82)thousand yuan of the control group.All the differences were statistically significant(P<0.05).Conclusion The value of the transparent anal tube display in laparoscopic Parks operation for rectal cancer is particularly outstanding.Through the application and improvement of technical means,the use of the transparent anal tube display during the anal operation in the perineal region has led to further improvement of the operation and better therapeutic results in terms of operation time,postoperative related indicators and prognosis of patients,which is worthy of vigoro
