摘  要:目的探讨TBC1结构域家族成员15蛋白(TBC1D15)、最小表观扩散系数(ADC)对脑胶质瘤恶性程度及预后的预测价值。方法选取132例脑胶质瘤患者,其中低级别胶质瘤患者56例,高级别胶质瘤患者76例,分析不同分级脑胶质瘤组织中TBC1D15蛋白、最小ADC值差异。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估TBC1D15、最小ADC值对脑胶质瘤恶性程度及预后的预测价值。结果高级别脑胶质瘤组织中TBC1D15蛋白积分光密度(IOD)明显高于低级别脑胶质瘤组织(P﹤0.01),最小ADC值明显低于低级别脑胶质瘤组织(P﹤0.01)。截至2021年6月,132例脑胶质瘤患者中,死亡52例,生存80例,死亡脑胶质瘤患者脑胶质瘤组织中TBC1D15蛋白IOD值明显高于生存患者(P﹤0.01),最小ADC值明显低于生存患者(P﹤0.01)。TBC1D15蛋白IOD值、最小ADC值预测高级别脑胶质瘤的AUC分别为0.883和0.848,此时的最佳截断值为0.99和1.11×10^(-3) mm^(2)/s。TBC1D15蛋白IOD值、最小ADC值预测脑胶质瘤患者死亡的AUC分别为0.913和0.888,此时的最佳截断值分别为1.12和0.95×10^(-3) mm^(2)/s。结论高级别脑胶质瘤组织TBC1D15蛋白表达上调,但最小ADC值较低,在预测脑胶质瘤恶性程度及预后方面有一定价值。Objective To explore the predictive value of TBC domain family member 15(TBC1D15)and minimum apparent diffusion coefficient(ADC)on the malignant degree and prognosis of glioma.Method Selected 132 patients with glioma,including 56 low-grade glioma patients and 76 high-grade glioma patients,and analyzed the differences of TBC1D15 protein and minimum ADC value in different grades of glioma tissue.Draw the receiver operating characteristic(ROC)curve,calculate the area under the curve(AUC),and evaluate the predictive value of TBC1D15 and the minimum ADC value for the malignant degree and prognosis of glioma.Result The integral optic density(IOD)of TBC1D15 protein in high-grade glioma tissue was significantly higher than that in low-grade glioma tissue(P<0.01),and the minimum ADC value was significantly lower than that in low-grade glioma tissue(P<0.01).By June 2021,among 132 patients with glioma,52 had died and 80 had survived.The IOD value of TBC1D15 protein in glioma tissue of dead patients was significantly higher than that of surviving patients(P<0.01),and the minimum ADC value was significantly lower than that of surviving patients(P<0.01).The TBC1D15 protein IOD value and the minimum ADC value predict the AUC of high-grade glioma to be 0.883 and 0.848,respectively,and the best cut-off values at this time are 0.99 and 1.11×10^(-3) mm^(2)/s.The IOD value of TBC1D15 protein and the minimum ADC value predicted the AUC of death of glioma patients were 0.913 and 0.888,respectively,and the best cut-off values at this time were 1.12 and 0.95×10^(-3) mm^(2)/s,respectively.Conclusion The expression of TBC1D15 protein in high-grade glioma tissue is up-regulated,but minimum ADC value is low,which has certain value in predicting the malignant degree and prognosis of glioma.
