摘 要:目的探讨超声诊断联合血清CA125、hs-CRP评定子宫内膜癌的价值。方法采用便利抽样法随机选取子宫内膜癌患者86例,为子宫内膜癌组;同期再选取子宫内膜良性病变患者90例,为良性病变组。观察和比较两组血清CA125、hs-CRP水平和超声检查及术后病理组织学结果。结果术后病理组织学检查资料显示子宫内膜癌86例,其中Ⅰ期34例、Ⅱ期27例、Ⅲ期18例、Ⅳ期7例;良性病变90例,其中子宫内膜息肉51例、黏膜下肌瘤24例、内膜增生12例、其他3例。超声诊断、超声诊断+CA125阳性、超声诊断+hs-CRP阳性符合率48.84%、74.42%、61.63%,均低于术后病理组织学检查的100.00%,差异有统计学意义(P<0.05);超声诊断+CA125阳性+hs-CRP阳性符合率93.02%低于术后病理组织学检查的100.00%,但差异无统计学意义(P>0.05)。超声诊断、超声诊断+CA125阳性、超声诊断+hs-CRP阳性、超声诊断+CA125阳性+hs-CRP阳性早期评定子宫内膜癌曲线下面积(AUC)分别为0.580、0.714、0.790、0.829。超声诊断+CA125阳性+hs-CRP阳性早期评定子宫内膜癌敏感度、特异性、阳性预测值、阴性预测值均高于单一项目。结论超声可对疑似病例进行有效识别,联合血清CA125、hs-CRP水平检测有助于进一步提升评定效果,可作为子宫内膜癌早期初步筛查首选方法。Objective To investigate the value of ultrasound diagnosis combined with serum CA125 and hs-CRP in evaluating endometrial cancer.Methods The convenience sampling method was used to randomly select 86 patients with endometrial cancer as the endometrial cancer group;at the same time,90 patients with benign endometrial lesions were selected as the benign lesion group.The serum CA125 and hs-CRP levels,ultrasound examination and postoperative histopathological results of the 2 groups were observed and compared.Results Postoperative histopathological examination data showed 86 cases of endometrial cancer,including 34 cases of stage I,27 cases of stage II,18 cases of stage III,and 7 cases of stage IV;90 cases of benign lesions,including 51 cases of endometrial polyps,there were 24 cases of submucosal fibroids,12 cases of intimal hyperplasia,and 3 other cases.The coincidence rates of ultrasound diagnosis,ultrasound diagnosis+CA125 positive,and ultrasound diagnosis+hs-CRP positive were 48.84%,74.42%,61.63%,which were all lower than 100.00%of postoperative histopathological examinations,and the difference was statistically significant(P<0.05).The coincidence rate of ultrasound diagnosis+CA125 positive+hs-CRP positive was 93.02%lower than 100.00%of postoperative histopathological examination,but the difference was not statistically significant(P>0.05).Ultrasound diagnosis,ultrasound diagnosis+CA125 positive,ultrasound diagnosis+hs-CRP positive,ultrasound diagnosis+CA125 positive+hs-CRP positive early assessment of endometrial cancer area under the curve(AUC)were 0.580,0.714,0.790,0.829,respectively.Ultrasound diagnosis+CA125 positive+hs-CRP positive early assessment of endometrial cancer sensitivity,specificity,positive predictive value,negative predictive value were higher than single items.Conclusion Ultrasound can effectively identify suspected cases.Combined with the detection of serum CA125 and hs-CRP levels can further improve the evaluation effect,and can be used as the first choice for early screening of endometrial
