摘  要:目的观察不同分级脑垂体瘤患者多层螺旋CT全脑灌注的成像特点,并分析经多层螺旋CT全脑灌注成像特点鉴别诊断不同分级脑垂体瘤的价值。方法选取治疗的41例脑垂体瘤患者,全部患者均接受多层螺旋CT全脑灌注成像检查,记录并比较患者病灶区及对侧正常区的多层螺旋CT全脑灌注成像参数[脑血流量(CBF)、脑容流量(CBV)、血管通透性(PMB)];依据不同分级将患者分为一至五级5组,对比不同分级脑垂体瘤与正常区域的多层螺旋CT全脑灌注成像参数,绘制受试者工作曲线(ROC),检验多层螺旋CT全脑灌注成像参数分别对不同分级脑垂体瘤的诊断价值。结果病灶区CBF、CBV及PMB值均大于对侧正常区,差异有统计学意义(P<0.05);研究纳入的41例脑垂体瘤患者中,一级至五级脑垂体瘤患者分别有10、8、11、9、3例;六组中,五级脑垂体瘤CBF、CBV及PMB值最大,其次为三、四级脑垂体瘤,对侧正常区CBF、CBV及PMB值最小,差异有统计学意义(P<0.05);绘制ROC曲线,多层螺旋CT全脑灌注成像参数诊断不同分级脑垂体瘤的AUC均>0.80,诊断价值较理想,且以联合诊断五级脑垂体瘤价值最高。结论不同分级脑垂体瘤的多层螺旋CT全脑灌注成像特点各异,临床可经多层螺旋CT全脑灌注成像所得参数鉴别不同分级脑垂体瘤,以提高分级诊断的准确率。Objective To observe the imaging characteristics of whole brain perfusion with multi-slice spiral CT in patients with different grades of pituitary tumors,and to analyze the value of whole brain perfusion imaging with multi-slice spiral CT in differential diagnosis of different grades of pituitary tumors.Methods 41 treated patients with pituitary adenoma were selected,all patients underwent the examination of whole brain perfusion imaging with multi-slice spiral CT,the parameters of whole brain perfusion imaging with multi-slice spiral CT in the lesion area and contralateral normal area of patients[cerebral blood flow(CBF),cerebral volume flow(CBV),vascular permeability(PMB)]were recorded and compared;the patients were divided into 5 groups according to different grades,the parameters of whole brain perfusion imaging with multi-slice spiral CT of different grades of pituitary tumors and normal areas were compared,the receiver operating curve(ROC)was drawn,the diagnostic value of the parameters of whole brain perfusion imaging with multi-slice spiral CT in different grades of pituitary tumors were tested.Results The values of CBF,CBV and PMB in lesion area were higher than those in contralateral normal area,with a statistical significant difference(P<0.05);among 41 patients with pituitary adenomas included in this study,there were 10,8,11,9 and 3 patients with grade 1 to 5 pituitary adenomas,respectively;in the 6 groups,the CBF,CBV and PMB values of grade 5 pituitary adenomas were the largest,followed by grade 3 and 4 pituitary tumors,and the CBF,CBV and PMB values in the contralateral normal area were the lowest,with statistical significant differences(P<0.05);the ROC curve was drawn,the AUC of parameters of whole brain perfusion imaging with multi-slice spiral CT in the diagnosis of pituitary tumors of different grades were all>0.80,the diagnostic value was ideal,and the combined diagnosis of grade 5 pituitary adenoma had the highest value.Conclusion The characteristics of whole brain perfusion imaging with mult
