摘 要:目的探讨磁共振波谱(MRS)技术在颅脑肿瘤鉴别诊断中的应用价值。方法回顾性分析48例颅脑肿瘤患者临床资料,入选者均经手术病理确诊,均进行MRS检查,分别计算N-乙酰天冬氨酸/胆碱(NAA/Cho)、Cho/肌酸(Cho/Cr)、NAA/Cr的均值。结果较正常脑组织,颅脑肿瘤病变区NAA均降低,CHO均升高,Cr趋于正常或稍降低;脑膜瘤各参数变化与转移瘤相比,Cho/Cr值较高,NAA/Cho值较低;转移瘤Cho/Cr值显著高于正常脑组织,NAA/Cr、NAA/Cho显著低于正常脑组织;胶质瘤NAA/Cr、NAA/Cho值对比,正常脑组织>Ⅰ~Ⅱ级胶质瘤>Ⅲ~Ⅳ级胶质瘤;Cho/Cr值相比,正常脑组织<Ⅰ~Ⅱ级胶质瘤<Ⅲ~Ⅳ级胶质瘤。垂体腺瘤各参数变化与淋巴瘤类似,但Cho/Cr值更高;淋巴瘤与正常脑组织相比,Cho/Cr值更高;星形细胞瘤各参数变化与胶质瘤类似,同级别NAA/Cr、NAA/Cho值相比,星形细胞瘤高于胶质瘤;同级别Cho/Cr相比,星形细胞瘤低于胶质瘤。结论颅脑肿瘤患者采用MRS技术诊断利于鉴别各肿瘤类型,为临床拟定治疗方案提供准确参考,在颅脑肿瘤诊断、鉴别中具有重要价值。Objective To explore the application value of magnetic resonance spectroscopy(MRS)in differential diagnosis of craniocerebral tumors.Methods The clinical data of 48 patients with craniocerebral tumor were retrospectively analyzed,All the patients were pathologically diagnosed by surgery,All MRS checks,N-mean values of acetylaspartate/choline(NAA/Cho),cho/creatine(Cho/Cr),naa/cr were calculated,respectively.Results All NAA in craniocerebral tumor area were decreased,All CHO are elevated,Cr tends to normal or slightly lower;The variation of parameters of meningioma compared with metastatic tumor had,high Cho/Cr,(a);low NAA/Cho values;Cho/Cr metastatic tumor was significantly higher than normal brain tissue,NAA/Cr、NAA/Cho significantly lower than normal brain tissue;Comparison NAA/Cr、NAA/Cho glioma values,Normal brain tissue>gradeⅠ~Ⅱglioma>gradeⅢ~Ⅳglioma;Compared to Cho/Cr values,Normal brain tissue<gradeⅠ~Ⅱglioma<gradeⅢ~Ⅳglioma.The parameters of pituitary adenoma were similar to those of lymphoma,But the Cho/Cr value is higher;Lymphoma compared to normal brain tissue,Higher Cho/Cr value;The parameters of astrocytoma are similar to those of glioma,Compared to NAA/Cr、NAA/Cho level,Astrocytoma is higher than glioma;Compared to Cho/Cr level,astrocytomas are lower than gliomas.Conclusion The diagnosis of craniocerebral tumor by MRS technique is helpful to differentiate each tumor type and provide accurate reference for clinical treatment.
