摘  要:目的探讨转录因子GATA结合蛋白4(GATA4)、微小RNA-195(miR-195)在恶性脑膜瘤组织中表达水平,分析其与预后的关系。方法选取文昌市人民医院自2015年5月至2019年5月收治的110例恶性脑膜瘤患者,取脑膜瘤组织、瘤旁组织检测GATA4、miR-195表达情况,并分析不同GATA4、miR-195表达情况患者临床病理特征。通过COX单因素及多因素模型分析影响恶性脑膜瘤患者预后的危险因素。结果脑膜瘤组织中GATA4阳性表达率高于瘤旁组织,miR-195表达量低于瘤旁组织,差异有统计学意义(P<0.05)。GATA4阳性患者WHO病理Ⅲ级、颅外转移占比高于GATA4阴性患者,差异有统计学意义(P<0.05)。miR-195高表达患者WHO病理Ⅲ级、颅外转移占比低于miR-195低表达患者,差异有统计学意义(P<0.05)。COX多因素分析显示,WHO病理Ⅲ级、颅外转移、GATA4阳性是患者预后的危险因素,miR-195高表达是患者预后的保护性因素(P<0.05)。结论恶性脑膜瘤组织中转录因子GATA4以阳性表达为主,miR-195表达则明显下调,WHO病理Ⅲ级、颅外转移、GATA4阳性是患者预后的危险因素,miR-195高表达是患者预后的保护性因素。Objective To investigate the expression levels of transcription factors GATA-binding protein 4(GATA4)and microRNA-195(miR-195)in malignant meningioma tissues,and analyze their effect on prognosis.Methods A retrospective study was performed on 110 patients with malignant meningioma admitted to Wenchang People′s Hospital from May 2015 to May 2019.Meningioma tissues and adjacent tissues were taken to detect the expression levels of GATA4 and miR-195,and clinicopathological characteristics of patients with different GATA4 and miR-195 expression were analyzed.The COX univariate and multivariate models were used to analyze the risk factors affecting the prognosis of patients with malignant meningioma.Results The positive expression rate of GATA4 in meningioma tissues was higher than that in adjacent tissues,and the expression level of miR-195 in meningioma tissues was lower than that in adjacent tissues,the differences were statistically significant(P<0.05).The proportions of WHO pathology gradeⅢand extracranial metastases in GATA4-positive patients were higher than those in GATA4-negative patients,the difference was statistically significant(P<0.05).The proportions of WHO pathology gradeⅢand extracranial metastases in patients with high miR-195 expression were lower than those in patients with low expression,the difference was statistically significant(P<0.05).COX multivariate analysis showed that WHO pathology gradeⅢ,extracranial metastasis,and GATA4 positive were the risk factors for the prognosis,and high expression of miR-195 was a protective factor for the prognosis(P<0.05).Conclusion The transcription factor GATA4 in malignant meningioma tissues is mainly positively expressed,while the expression level of miR-195 is significantly down-regulated.WHO pathology gradeⅢ,extracranial metastasis,and GATA4 positive are risk factors for the prognosis of patients,and high expression of miR-195 is the protective factor for the prognosis of patients.
