HPV阳性宫颈癌组织中miR-34b的表达水平及其临床意义

摘　　要：目的:探讨miR-34b在HPV阳性宫颈癌组织中的表达水平及其对患者的临床意义。方法:收集65例HPV阳性宫颈癌组织标本和35例正常宫颈组织标本,采用Real-time PCR法测定miR-34b的相对表达水平,分析其与患者临床病理特征的关系以及对患者诊断、预后的价值。结果:miR-34b在HPV阳性宫颈癌组织中的相对表达量明显低于正常宫颈组织,差异具有统计学意义(t=9.549,P<0.05);在HPV阳性宫颈癌患者中,miR-34b的相对表达量与患者的年龄、病理类型、肿瘤分级均无明显关系(P>0.05),与患者的肿瘤大小、FIGO分期、淋巴结是否转移显著相关,差异具有统计学意义(χ^(2)=4.996、5.925、5.824,P<0.05);进行ROC曲线分析,发现miR-34b区分HPV阳性宫颈癌细胞与正常宫颈细胞的AUC为0.871(95%CI:0.773~0.970,P=0.000),灵敏度为90.24%,特异度为79.17%;对HPV阳性宫颈癌患者随访3年,发现miR-34b高表达患者的OS、DFS均显著高于低表达患者,差异具有统计学意义(χ^(2)=5.856、5.919,P<0.05)。结论:miR-34b在HPV阳性宫颈癌组织中呈低表达,可能为HPV阳性宫颈癌筛查、诊断和治疗提供帮助,具体情况还有待进一步研究。Objective:To investigate the expression level of miR-34b in HPV-positive cervical cancer and its clinical value.Methods:65 HPV-positive cervical cancer tissue specimens and 35 normal cervical tissue specimens were collected.The relative expression level of miR-34b was determined by Real-time PCR,and its relationship with clinicopathological features and the value of diagnosis and prognosis were analyzed.Results:The relative expression of miR-34b in HPV-positive cervical cancer tissue was significantly lower than that in normal cervical tissue(t=9.549,P<0.05).In HPV-positive cervical cancer patients,the relative expression of miR-34b was not significantly related to age,pathological type and tumor grade(P>0.05),but was significantly related to tumor size,FIGO stage and lymph node metastasis(χ^(2)=4.996,5.925,5.824,P<0.05).The ROC curve analysis showed that the AUC of miR-34b to differentiate HPV-positive cervical cancer cells from normal cervical cells was 0.8714(95%CI:0.773~0.970,P=0.000),the sensitivity and specificity was 90.24%and 79.17%,respectly.Follow-up of HPV positive cervical cancer patients for 3 years showed that OS and DFS of patients with high expression of miR-34b were significantly higher than those with low expression(χ^(2)=5.856,5.919,P<0.05).Conclusion:MicroRNA-34b is low expression in HPV positive cervical cancer,which may provide help for screening,diagnosis and treatment of HPV positive cervical cancer.The specific situation needs further study.
