食管癌根治术后肺部感染危险因素及肺功能改变的探讨

摘　　要：目的分析食管癌根治术后肺部感染危险因素及肺功能改变。方法选择2017年3月至2020年3月于我院行食管癌根治术的300例患者,比较其术后肺部感染情况,分析食管癌根治术后肺部感染的危险因素及肺功能相关指标。结果300例食管癌患者中,39例(13.0%)发生术后肺部感染。影响食管癌患者术后发生肺部感染的单因素为年龄、长期吸烟史、术前肺功能、合并糖尿病、手术方式、手术时长(P<0.05)。影响食管癌患者术后发生肺部感染的独立风险因素为年龄、长期吸烟史、术前肺功能、合并糖尿病及手术方式(P<0.05)。结论年龄、长期吸烟史、术前肺功能、合并糖尿病及手术方式是影响食管癌患者术后发生肺部感染的独立风险因素,肺部感染会使食管癌患者发生肺功能降低的风险明显升高。Objective To analyze the risk factors of lung infection and the changes of lung function after radical resection of esophageal cancer.Methods 300 patients with radical resection of esophageal cancer in our hospital from March 2017 to March 2020 were selected.The postoperative lung infection was compared,and the risk factors of lung infection and lung function related indicators were analyzed.Results Among 300 patients with esophageal cancer,39 cases(13.0%)had postoperative lung infection.The single factors that affected postoperative lung infection in patients with esophageal cancer were age,long-term smoking history,preoperative lung function,diabetes,surgical method,and surgical duration(P<0.05).The independent risk factors that affected postoperative lung infections in patients with esophageal cancer were age,long-term smoking history,preoperative lung function,diabetes,and surgical method(P<0.05).Conclusions Age,long-term smoking history,preoperative lung function,diabetes,and surgical method are independent risk factors that affect postoperative lung infection in patients with esophageal cancer.Lung infection will significantly increase the risk of postoperative lung function decline in patients with esophageal cancer.
