通腑醒脑汤结合静脉溶栓治疗在急性脑梗死患者中的疗效研究

摘　　要：目的探究通腑醒脑汤结合静脉溶栓治疗在急性脑梗死患者中的疗效。方法随机数字法将医院治疗急性脑梗死患者156例分为静脉溶栓组(intravenous thrombolysis group,IT组)78例、通腑醒脑汤联合静脉溶栓组(Tongfu Xingnao decoction combined with intravenous thrombolysis group,TI组)78例,病例来源时间:2018年7月-2020年6月,比较IT组和TI组患者治疗效果,记录ADL与NIHSS评分、TXB2、AT-Ⅲ及炎性水平的差异性。结果经过治疗后TI组的治疗总有效率(96.15%)高于IT组治疗总有效率(84.62%),组间差异有统计学意义(P <0.05);治疗前两组ADL评分与NIHSS评分分数较为接近(P> 0.05);经过治疗后TI组ADL评分较IT组高、NIHSS评分较IT组低(P <0.05);治疗前两组TXB2、AT-Ⅲ水平较为接近(P> 0.05);经过治疗后,TI组TXB2指标水平低于IT组、AT-Ⅲ的水平高于IT组(P <0.05);治疗前两组患者IL-6、TNF-α、CRP水平较为接近(P> 0.05);经过治疗后,TI组IL-6、TNF-α、CRP水平均较IT组降低(P <0.05)。结论通腑醒脑汤结合静脉溶栓对急性脑梗死患者治疗效果较好,可提高生活能力,降低神经损伤,改善TXB2、AT-Ⅲ及炎性水平,对于急性脑梗死治疗有重要意义。Objective To explore the effect of Tongfu Xingnao decoction combined with intravenous thrombolysis in patients with acute cerebral infarction. Methods Random number method was used to divide 156 patients with acute cerebral infarction in hospital into intravenous thrombolysis group(IT group) 78 patients, Tongfu Xingnao decoction combined with intravenous thrombolysis group( TI group) 78 cases, case source time: from July 2018 to June 2020, compared the treatment effects of patients in the IT group and the TI group, and recorded the differences in ADL and NIHSS scores, TXB2, AT-Ⅲ, and inflammatory levels. Results After treatment, the total effective rate of TI group(96.15%) was higher than that of IT group(84.62%), and the difference between the two groups was statistically significant(P < 0.05);before treatment, the ADL score of the two groups was close to the NIHSS score(P > 0.05);after treatment, the ADL score of TI group was higher than that of IT group, and the NIHSS score was lower than that of IT group(P < 0.05);before treatment, the levels of TXB2 and AT-Ⅲ of the two groups were close(P > 0.05). After treatment, the level of TXB2 in TI group was lower than that in IT group, and the level of AT-Ⅲ was higher than that in IT group(P < 0.05);before treatment, the levels of IL-6, TNF-α and CRP in the two groups were close(P > 0.05);after treatment, the levels of IL-6, TNF-α and CRP in TI group were lower than those in IT group(P < 0.05). Conclusion Tongfu Xingnao decoction combined with intravenous thrombolysis has a good therapeutic effect on patients with acute cerebral infarction. It can improve life ability,reduce nerve injury, improve TXB2, AT-Ⅲ and inflammatory levels, which is of great significance for the treatment of acute cerebral infarction.
