联合方式的小梁切除术治疗新生血管性青光眼临床疗效分析

摘　　要：目的:评估康柏西普注射联合交联透明质酸钠辅助小梁切除手术治疗新生血管性青光眼(NVG)的临床疗效。方法:2016年7月-2018年10月收治NVG患者17例(17眼),均术前3 d注射康柏西普于患眼玻璃体腔,按照治疗方法分为两组。对照组应用单纯小梁切除术;试验组应用交联透明质酸钠辅助小梁切除术。比较两组临床疗效。结果:试验组术后6个月最佳矫正视力(BCVA)改善效果优于对照组,差异有统计学意义(P<0.05);试验组治疗后3个月及6个月眼压水平均低于对照组,差异有统计学意义(P<0.05);试验组术后3个月及6个月功能性滤过泡多于对照组,差异有统计学意义(P<0.05);术后6个月,试验组手术成功率高于对照组,差异有统计学意义(P<0.05)。结论:康柏西普联合交联透明质酸钠辅助小梁切除术能有效降低NVG患者的眼压水平,且能有效维持视功能。Objective:To evaluate the clinical efficacy of trabeculectomy assisted by cross-linked sodium hyaluronate after conbercept injection in the treatment of neovascular glaucoma(NVG).Methods:From July 2016 to October 2018,17 patients(17 eyes)with NVG were selected.All of them were injected with conbercept into the vitreous cavity of the affected eye 3 days before the operation.They were divided into two groups according to the treatment method.The control group was treated with simple trabeculectomy.The experimental group was treated with cross-linked sodium hyaluronate-assisted trabeculectomy.We compared the clinical efficacy of the two groups.Results:The best corrected visual acuity(BCVA)of the experimental group was better than that of the control group at 6 months after operation,the difference was statistically significant(P<0.05).The intraocular pressure levels of the experimental group were lower than those of the control group at 3 months and 6 months after treatment,the difference was statistically significant(P<0.05).There were more functional filtering blebs in the experimental group than in the control group at 3 and 6 months after operation,the difference was statistically significant(P<0.05).At 6 months after operation,the success rate of surgery in the experimental group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Trabeculectomy assisted by conbercept combined with cross-linked sodium hyaluronate can effectively reduce the intraocular pressure of patients with NVG and can effectively maintain visual function.
