^(18)F-FDG PET/CT在早期宫颈癌盆腔淋巴结转移中的诊断价值

摘　　要：目的:探讨^(18)F-FDG PET/CT在早期宫颈癌盆腔淋巴结转移中的诊断价值。方法:2015年1月-2019年12月收治宫颈癌患者90例,以病理结果为金标准,计算^(18)F-FDG PET/CT在盆腔淋巴结转移的准确率、灵敏度及特异度,并比较有无淋巴结转移患者间的代谢参数差异。结果:90例患者中,经病理证实有盆腔淋巴结转移23例,无淋巴结转移67例;^(18)F-FDG PET/CT在盆腔淋巴结转移的准确率、灵敏度及特异度分别为77.78%、54.05%、94.34%。有淋巴结转移患者糖酵解总量(TLG)及肿瘤代谢体积(MTV)参数均明显高于无淋巴结转移患者,差异有统计学意义(P<0.05);有淋巴结转移患者与无淋巴结转移患者最大SUV(SUVmax)及平均SUV(SUVave)参数比较,差异无统计学意义(P>0.05)。结论:^(18)F-FDG PET/CT在早期宫颈癌盆腔淋巴结转移中有较高的特异性,TLG及MTV也有一定的诊断价值。Objective:To explore the diagnostic value of^(18)F-FDG PET/CT in pelvic lymph node metastasis of early cervical cancer.Methods:From January 2015 to December 2019,90 patients with cervical cancer were selected,pathological results were regarded as the gold standard,we calculated the accuracy,sensitivity and specificity of^(18)F-FDG PET/CT in pelvic lymph node metastasis,the differences of metabolic parameters between patients with and without lymph node metastasis were compared.Results:Of the 90 patients,there were 23 cases with pelvic lymph node metastasis and 67 cases without lymph node metastasis confirmed by pathology.The accuracy,sensitivity and specificity of PET/CT in pelvic lymph node metastasis were 77.78%,54.05%and 94.34%respectively.The parameters of TLG and MTV in patients with lymph node metastasis were significantly higher than those in patients without lymph node metastasis,the difference was statistically significant(P<0.05).There was no significant difference in SUVmax and SUVave parameters between the group with lymph node metastasis and group without lymph node metastasis(P>0.05).Conclusion:^(18)F-FDG PET/CT has high specificity in pelvic lymph node metastasis of early cervical cancer,TLG and MTV also have certain diagnostic value.
