摘  要:目的:采用三维有限元分析不同程度重度缺损的上颌中切牙使用不同材料个性化桩核修复后的应力分布情况。方法:利用微焦点CT及三维有限元软件,建立上颌中切牙重度缺损有限元模型,缺损分为2 mm高的有牙本质肩领和平齐根面的无牙本质肩领组,分别模拟使用纯钛桩核、铸造金合金桩核、纤维增强树脂桩核3种桩核材料进行修复建模。结果:随着桩弹性模量增大,牙本质应力减小,但分布范围无差异;牙本质肩领的缺失可能会使应力大小不稳定。结论:有无肩领及不同材料对剩余牙本质的Von Mises应力和最大主应力分布影响不大。牙本质肩领组的剩余牙体组织应力明显小于无肩领组,利于减少根折。Objective:To study the stress distribution of maxillary central incisors with different degrees of severe defects restored by custom post-and-core with different materials by a 3D finite element analysis.Methods:Using Micro CT and three-dimensional finite element software,a finite element model of a maxillary central incisor with severe defect was established.The samples were divided into 2 groups:with 2 mm high dentin ferrule and without dentin ferrule,then the restorations were reconstructed using 3 types of materials:cast titanium,cast gold alloy and fiber-reinforced composite resin,respectively.Results:The dentin stress decreased as the elastic modulus of the post increased,but the distribution range remained the same.Stress levels became unstable in the model without the dentine ferrule.Conclusion:The distribution of Von Mises stress and maximum principal stress of residual dentin was not affected by the presence of ferrule or materials.The stress in the dentin ferrule group was lower than that in the without ferrule group,which was beneficial to reduce root fractures.
