摘 要:目的:探讨上颌第二恒磨牙根管预备后锥度和宽度与牙根抗压性能的相关性。方法:收集因正畸新鲜拔除的上颌第二恒磨牙91颗,离体牙截冠后进行根管预备、侧方加压充填根管,并进行疲劳实验和力学实验。根据疲劳实验后是否出现裂纹分为有裂纹组和无裂纹组。结果:61例无裂纹组,30例有裂纹组,裂纹多为纵向裂纹,颊舌面裂纹多于近远中面,集中于根中上1/3处;有裂纹组根管口直径、根尖孔直径、锥度均高于无裂纹组,最大载荷低于无裂纹组(P<0.05);根管口直径、根尖孔直径、锥度与最大载荷呈负相关(r=-0.680、-0.686、-0.669,P均<0.05);根管口直径、根尖孔直径、锥度在预测牙根抗压性能方面均具有良好效能,其中根管口直径预测的曲线下面积(AUC)最大,为0.785,最佳预测敏感度为83.33%、特异度为62.30%。结论:上颌第二恒磨牙根管预备后锥度和宽度与牙根抗压性能呈明显负相关,且根管口直径、根尖孔直径、锥度在预测牙根抗压性能方面均具有良好效能。Objective:To investigate the relationship between the taper and width of the root canal of the maxillary second permanent molar after root canal preparation and the compression resistance of the root.Methods:91 maxillary second permanent molars freshly extracted due to orthodontics were selected.After removal of the crowns of the teeth,the root canals were prepared,and filled by lateral pressure.The fatigue experiments and mechanical experiments were carried out.According to whether cracks appeared after the fatigue test,the samples were divided into cracked group and non-cracked group.Results:There were 61 teeth in non-cracked group and 30 in cracked group.The cracks in the cracked group were mostly longitudinal.The buccal and lingual cracks were more than the mesiodistal,concentrated in the upper third of the roots.The apical foramen diameter,canal orifice diameter and taper of the cracked group were higher than those of the non-cracked group(P<0.05),and the maximum load was lower than that of the non-cracked group(P<0.05).The diameter of the root canal orifice,that of the apical foramen,and the taper were negatively correlated with the maximum load(r=-0.680,-0.686 and-0.669 respectively,all P<0.05).The canal orifice diameter,apical foramine diameter,and taper were all effective in predicting the compressive performance of tooth roots.The area under the curve(AUC)predicted by root canal orifice diameter was the largest,0.785,the best prediction sensitivity was 83.33%,and the specificity was 62.30%.Conclusion:The taper and width of the root canal of the maxillary second permanent molar after preparation are significantly negatively correlated with the root compression performance.The root canal orifice diameter,apical foramen diameter and taper are all effective in predicting the compressive performance of tooth roots.
