摘　　要：目的探讨经肝动脉化疗栓塞联合射频消融术对原发性肝癌患者免疫功能及血清甲胎蛋白(AFP)、糖类抗原199(CA-199)、血管内皮细胞生长因子(VEGF)水平的影响。方法选取2018年8月至2019年5月我院收治的90例原发性肝癌患者作为研究对象,按照随机数字表法将其分为对照组和试验组,每组45例。对照组采用经肝动脉化疗栓塞治疗,试验组采用经肝动脉化疗栓塞联合射频消融术治疗。比较两组术后1个月的临床疗效,术前、术后2周的免疫功能及血清AFP、CA-199、VEGF水平。结果试验组术后1个月的治疗总有效率为88.89%,高于对照组的68.89%(P<0.05)。术后2周,两组的CD4^(+)、CD4^(+)/CD8^(+)均升高,CD8^(+)均降低,且试验组优于对照组(P<0.05)。术后2周,两组的血清AFP、CA-199、VEGF水平均降低,且试验组低于对照组(P<0.05)。结论经肝动脉化疗栓塞联合射频消融术治疗原发性肝癌的效果显著,可改善患者的免疫功能,抑制肿瘤进一步扩散。Objective To investigate the effects of transcatheter arterial chemoembolization combined with radiofrequency ablation on immune function and serum alpha fetoprotein(AFP),carbohydrate antigen-199(CA-199)and vascular endothelial growth factor(VEGF)levels in patients with primary liver cancer.Methods A total of 90 patients with primary liver cancer admitted in our hospital from August 2018 to May 2019 were selected as the research objects.The patients were divided into control group and experimental group according to random number table method,with 45 cases in each group.The control group was treated with transcatheter arterial chemoembolization,and the experimental group was treated with transcatheter arterial chemoembolization combined with radiofrequency ablation.The clinical efficacy at one month after operation,immune function and the levels of serum AFP,CA-199 and VEGF before and 2 weeks after operation were compared between the two groups.Results The total effective rate of treatment at one month after operation in the experimental group was 88.89%,which was higher than 68.89%in the control group(P<0.05).Two weeks after operation,CD4^(+),CD4^(+)/CD8^(+)in the two groups increased,CD8^(+)decreased,and those in the experimental group were better than the control group(P<0.05).Two weeks after operation,the levels of serum AFP,CA-199 and VEGF in the two groups decreased,and those in the experimental group were lower than the control group(P<0.05).Conclusion Transcatheter arterial chemoembolization combined with radiofrequency ablation in the treatment of primary liver cancer has a significant effect.It can improve the immune function of patients and inhibit the further spread of tumor.
