血清ANCA与ASCA和GAB联合检测对炎症性肠病的临床价值

摘　　要：目的探讨血清抗中性粒细胞胞浆抗体(ANCA)与抗酿酒酵母抗体(ASCA)和抗小肠杯状细胞抗体(GAB)联合检测对炎症性肠病(IBD)的临床价值。方法选取本院2017年5月至2019年5月收治的IBD患者63例作为观察组,并选取同期健康体检者50名作为对照组,全部受检者均行血清ANCA、ASCA和GAB检测,比较两组检测阳性率及联合指标检测与单项指标检测诊断IBD的结果。结果观察组血清ANCA阳性率(71.43%)、ASCA阳性率(75.60%)、GAB阳性率(80.95%)均高于对照组(32.00%、26.00%、24.00%),差异有统计学意义(P<0.05);血清ANCA、ASCA、GAB联合检测诊断IBD特异性(100.00%)、敏感性(95.24%)、准确性(97.35%)均高于单项指标ANCA(64.00%、68.25%、66.37%)、ASCA(78.00%、69.84%、73.45%)、GAB(74.00%、79.37%、76.99%),差异有统计学意义(P<0.05)。结论血清ANCA、ASCA和GAB联合检测诊断IBD临床应用价值较高,值得临床推广运用。Objective To explore the clinical value of combined detection of serum anti-neutrophil cytoplasmic antibody(ANCA)with anti-saccharomyces cerevisiae antibody(ASCA)and anti-small intestinal goblet cell antibody(GAB)for inflammatory bowel disease(IBD).Methods 63 IBD patients treated in our hospital from May 2017 to May 2019 were selected as the observation group,and 50 healthy subjects were selected as the control group at the same time.All the subjects were tested for serum ANCA,ASCA and GAB.The positive rate of detection between the two groups and the results of combined index detection and single index detection in the diagnosis of IBD were compared.Results The serum ANCA positive rate(71.43%),ASCA positive rate(75.60%),and GAB positive rate(80.95%)in the observation group were higher than those in the control group(32.00%,26.00%,24.00%),the difference was statistically difference(P<0.05);the specificity(100.00%),sensitivity(95.24%),and accuracy(97.35%)of serum ANCA,ASCA,and GAB in the diagnosis of IBD were higher than the individual indicators ANCA(64.00%,68.25%,66.37%),ASCA(78.00%,69.84%,73.45%),GAB(74.00%,79.37%,76.99%),the difference was statistically difference(P<0.05).Conclusion The combined detection of serum ANCA,ASCA and GAB in diagnosis of IBD is reliable and has important clinical application value.
