摘 要:目的观察血管细胞黏附分子(VCAM-1)短期内水平变化与乳腺癌患者术后恢复和并发症的关系。方法选取本院2018年1月至2019年6月收治的行乳腺癌保乳手术联合腋窝淋巴结清扫术患者119例,按照患者术前VCAM-1水平分为观察组(<53.27 ng/mL,n=61)与对照组(≥53.27 ng/mL,n=58)。比较两组术前24 h、术后24 h、术后72 h、术后5 d外周静脉血VCAM-1水平及腋窝日引流量、腋窝引流管拔管时间、并发症发生率。结果术后24 h、72 h及术后5 d,两组VCAM-1水平均低于术前24 h(P<0.05),术后24、72 h,两组VCAM-1水平比较差异均无统计学意义,但术后5 d,观察组VCAM-1水平低于对照组(P<0.05)。观察组腋窝日引流量和腋窝拔管时间均低于对照组(P<0.05)。观察组术后并发症发生率低于对照组(P<0.05)。结论VCAM-1水平变化与乳腺癌腔镜腋窝淋巴结清扫术术后患者恢复状况和并发症发生率显著相关。Objective To observe the relationship between hanges of short-term cvascular cell adhesion molecule(VCAM-1)and postoperative recovery and complications in breast cancer patients.Methods 119 patients with breast cancer undergoing breast conservation surgery combined with axillary lymph node dissection in our hospital from January 2018 to June 2019 were selected.According to the preoperative VCAM-1 level,theywere divided into the observation group(<53.27 ng/mL,n=61)and the control group(≥53.27 ng/mL,n=58).The levels of VCAM-1 in peripheral venous blood,the daily axillary drainage volume,the extubation time of axillary drainage tube and the incidence of complications were compared between the two groups at 24 h before surgery,24 h after surgery,72 h after surgery and 5 d after surgery.Results At 24 h,72 h and 5 d after surgery,the level of VCAM-1 in the two groups was lower than 24 h before surgery(P<0.05).There was no significant difference in the level of VCAM-1 in the two groups at 24 h and 72 h after surgery,but at 5 d after surgery,the level of VCAM-1 in the observation group was lower than that in the control group(P<0.05).The daily axillary drainage volume and axillary extubation time in observation group were lower than those in control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion The change of VCAM-1 level is significantly correlated with the recovery and complication rate of patients after endoscopic axillary lymph node dissection for breast cancer.
