摘 要:目的:检测血清早期前列腺癌抗原-2(EPCA-2)与尿液前列腺癌抗原3(PCA3)在前列腺特异性抗原(PSA)灰区(4~10ng/mL)前列腺癌患者中的表达,探讨联合检测对早期诊断PSA灰区前列腺癌的应用价值。方法:将180例PSA灰区病例分为前列腺癌组(46例)和前列腺良性增生组(134例),同时选取健康体检者120例作为正常对照组,检测三组血清EPCA-2和尿液PCA3的表达,并分析联合检测EPCA-2和PCA3对鉴别PSA灰区前列腺癌与良性前列腺增生的诊断价值。结果:前列腺癌组与前列腺良性增生组血清EPCA-2水平均增高,与正常对照组比较,差异均有统计学意义(均P<0.05)。前列腺癌组29例尿液PCA3基因表达呈阳性,而前列腺良性增生组仅8例表达呈阳性,正常对照组PCA3基因表达均呈阴性。联合检测EPCA-2与PCA3鉴别PSA灰区前列腺癌和前列腺增生的敏感度为76.08%,特异度为85.82%。结论:联合检测血清EPCA-2与尿液PCA3可提高PSA灰区前列腺癌的早期诊断率。Objective:To detect the expression of serum early prostate cancer antigen-2(EPCA-2)and urine prostate cancer antigen 3(PCA3)in prostate cancer patients with prostate specific antigen(PSA)in grey zone(4 to 10 ng/mL),and explore application value of combined detection in early diagnosis of prostate cancer with PSA in grey zone.Methods:A total of 180 patients with PSA in grey zone were collected and divided into the prostate cancer group(46 cases)and the benign prostatic hyperplasia group(134 cases).There were 120 healthy persons were selected as the normal control group.The expressions of serum EPCA-2 and urine PCA3 were detected.The diagnostic value for combined detection of EPCA-2 and PCA3 in differentiating prostate cancer(PSA grey zone)from benign prostatic hyperplasia was analyzed.Results:Compared with the normal control group,the levels of serum EPCA-2 were significant increased both in the prostate cancer group and the benign prostatic hyperplasia group,with statistical differences(both P<0.05).There were 29 cases with PCA3 positive expression in the prostate cancer group,and 8 cases positive in the benign prostatic hyperplasia group,and all negative in the normal control group.The combined detection of EPCA-2 and PCA3 were 76.08% for sensitivity and 85.82% for specificity in differentiating prostate cancer(PSA grey zone)and benign prostatic hyperplasia.Conclusion:The combined detection of serum EPCA-2 and urine PCA3 can improve the early diagnosis rate of prostate cancer with PSA in grey zone.
