牙周基础治疗对侵袭性牙周炎患者龈沟液dickkopf相关蛋白1水平的影响

摘　　要：目的观察侵袭性牙周炎(aggressive periodontitis,AgP)患者牙周基础治疗前、后龈沟液dickkopf相关蛋白1(dickkopf-related protein 1,DKK1)水平变化,探讨其在AgP中的作用。方法AgP患者40例为AgP组,牙周健康者10例为对照组,2组均行超声龈上洁治,观察组洁治后1周行龈下刮治。观察组分别于治疗前、洁治后1周、刮治后6周,对照组分别于治疗前、洁治后1周评估探诊出血指数(bleeding index,BI)、牙周探诊深度(probing depth,PD)、临床附着水平(clinical attachment level,CAL),采用ELISA法检测龈沟液DKK1水平,并进行比较。结果AgP组BI、PD、CAL、龈沟液DKK1水平在洁治前[4.12±0.65、(5.98±0.57)mm、(5.98±0.88)mm、(9.02±0.23)μg/L]、洁治后1周[3.42±0.96、(4.95±0.43)mm、(5.88±1.10)mm、(8.55±0.64)μg/L]均高于对照组[洁治前:1.96±0.37、(1.81±0.32)mm、(0.54±0.11)mm、(5.54±2.45)μg/L;洁治后1周:1.87±0.34、(1.98±0.22)mm、(0.43±0.09)mm、(5.32±1.59)μg/L](P<0.05);AgP组刮治后6周BI(1.65±0.43)、PD[(3.96±0.98)mm]、龈沟液DKK1水平[(5.83±0.52)μg/L]均低于洁治后1周及洁治前(P<0.05),洁治后1周BI、PD均低于洁治前(P<0.05),洁治后1周龈沟液DKK1水平与洁治前比较差异无统计学意义(P>0.05),刮治后6周CAL与洁治后1周、洁治前比较差异均无统计学意义(P>0.05);对照组洁治后1周BI、PD、CAL、龈沟液DKK1水平与洁治前比较差异均无统计学意义(P>0.05)。结论AgP患者龈沟液DKK1水平升高,龈上洁治和龈下刮治可降低BI、PD,可能与牙周基础治疗后龈沟液DKK1水平降低有关。Objective To observe the changes of dickkopf-related protein 1(DKK1)level in gingival crevicular fluid(GCF)of patients with aggressive periodontitis(AgP)before and after periodontal therapy,and to explore the relationship between DKK1 level and AgP.Methods Forty patients with AgP(AgP group)and 10 healthy volunteers(control group)received ultrasonic supragingival scaling,and AgP group received subgingival scaling one week later.The bleeding index(BI),probing depth(PD)and clinical attachment level(CAL)were recorded before treatment,one week after supragingival scaling and six weeks after subgingival scaling in AgP group and before and one week after supragingival scaling in control group.The level of DKK1 was detected in gingival crevicular fluid by ELISA and was compared.Results The BI,PD,CAL and DKK1 level were high before treatment(4.12±0.65,(5.98±0.57)mm,(5.98±0.88)mm,(9.02±0.23)μg/L)and one week after supragingival scaling(3.42±0.96,(4.95±0.43)mm,(5.88±1.10)mm,(8.55±0.64)μg/L)in AgP group than those in control group(before scaling:1.96±0.37,(1.81±0.32)mm,(0.54±0.11)mm,(5.54±2.45)μg/L;one week after supragingival scaling:1.87±0.34,(1.98±0.22)mm,(0.43±0.09)mm,(5.32±1.59)μg/L)(P<0.05).The BI,PD and DKK1 level were lower six weeks after subgingival scaling(1.65±0.43,(3.96±0.98)mm,(5.83±0.52)μg/L)than those before treatment and one week after supragingival scaling in AgP group(P<0.05).BI and PD were lower one week after supragingival scaling than those before treatment(P<0.05),DKK1 level showed no significant difference one week after supragingival scaling compared with that before treatment(P>0.05),and CAL showed no significant difference six weeks after subgingival scaling compared with that before treatment and one week after supragingival scaling in AgP group(P>0.05).The BI,PD,CAL and DKK1 level one week after supragingival scaling showed no significant differences compared with those before treatment in control group(P>0.05).Conclusion The level of DKK1 is elevated in patients with AgP.
