摘 要:目的探讨核磁共振(MR)扩散加权成像(DWI)及表观弥散系数(ADC)图像在小儿颅内肿瘤分级诊断中的应用价值。方法选取2019年5月至2020年9月期间西安市儿童医院经临床确诊的80例颅内肿瘤患儿作为研究对象,按照患儿肿瘤分级程度分为Ⅰ~Ⅱ级组46例和Ⅲ~Ⅳ级组34例,分析两组患儿DWI、ADC图像信息,比较两组患儿肿瘤实质、正常脑白质的ADC值以及肿瘤实质/正常脑白质(rADC值)。结果Ⅰ~Ⅱ级组患儿的DWI、ADC图像中稍高信号、等信号占比均明显高于Ⅲ~Ⅳ级组,高信号占比明显低于Ⅲ~Ⅳ级组,差异均有统计学意义(P<0.05);Ⅰ~Ⅱ级组患儿的肿瘤实质ADC值、rADC值分别为(1.25±0.35)×10^(-3)mm^(2)/s、1.61±0.48,明显高于Ⅲ~Ⅳ级组的(0.89±0.26)×10^(-3)mm^(2)/s、1.19±0.35,差异均具有统计学意义(P<0.05);Ⅰ~Ⅱ级组患儿正常脑白质ADC值为(0.76±0.02)×10^(-3)mm^(2)/s与Ⅲ~Ⅳ级组的(0.77±0.09)×10^(-3)mm^(2)/s比较差异无统计学意义(P>0.05)。结论MR的DWI及ADC图像分析应用于小儿颅内肿瘤分级诊断中,能帮助临床医生及时有效的掌握患儿肿瘤分级情况,以指导临床有效治疗,对临床诊治具有实用价值。Objective To investigate the application value of magnetic resonance(MR)combined diffusion-weighted imaging(DWI)and apparent diffusion coefficient(ADC)in the grading diagnosis of intracranial tumors in children.Methods From May 2019 to September 2020,80 children with intracranial tumors clinically diagnosed in Xi’an Children’s Hospital were selected as the research objects.According to the tumor grade of the children,they were divided into gradeⅠ~Ⅱgroup(n=46)and gradeⅢ~Ⅳgroup(n=34).The DWI and ADC image information of the two groups were analyzed,and the ADC values of tumor parenchyma and normal white matter were compared,as well as the ration of tumor parenchyma/normal white matter(rADC value)between the two groups.Results The proportions of slightly high signal and equal signal in the DWI and ADC images of gradeⅠ~Ⅱgroup were significantly higher than those of gradeⅢ~Ⅳgroup,and the proportion of high signal was significantly lower than that of gradeⅢ~Ⅳgroup,with statistically significant differences(P<0.05).The ADC value and rADC value of tumor parenchyma in gradeⅠ~Ⅱgroup were(1.25±0.35)×10^(-3)mm^(2)/s and 1.61±0.48,respectively,which were significantly higher than(0.89±0.26)×10^(-3)mm^(2)/s and 1.19±0.35 in gradeⅢ~Ⅳgroup(P<0.05).The ADC value of normal white matter in gradeⅠ~Ⅱgroup was(0.76±0.02)×10^(-3)mm^(2)/s,and there was no statistically significant difference as compared with(0.77±0.09)×10^(-3)mm^(2)/s in gradeⅢ~Ⅳgroup(P>0.05).Conclusion The application of MR combined DWI and ADC image analysis in the grading diagnosis of intracranial tumors in children can help clinicians to grasp the tumor grading in children timely and effectively,and guide clinicians to carry out effective treatment for children,which has practical value for clinical diagnosis and treatment.
