摘  要:目的研究微生态免疫营养剂对脓毒症肠功能障碍患者的作用,为该类患者的治疗提供参考。方法选取我院2019年3月至2020年2月收治的140例脓毒症肠功能障碍患者为研究对象,所有患者随机分成研究组和常规组,每组70例。患者入院后实施常规营养支持干预,其中研究组对象联合应用微生态免疫营养剂,使用时间为14 d。对比两组患者的临床疗效及相关指标水平。结果治疗第7天和第14天时,所有患者的CRP、PCT水平均显著下降,且研究组该两项指标水平显著低于常规组(均P<0.05)。治疗前两组患者CD3+细胞、CD4+细胞、CD8+细胞、CD4+/CD8+、IgA、IgG和IgM水平差异均无统计学意义(均P>0.05)。治疗第7天和第14天时,研究组患者上述7项指标水平显著高于常规组(均P<0.05)。治疗第7天和第14天时,研究组患者粪便样本中有益菌数量高于常规组(均P<0.05)。治疗结束后,研究组患者治疗总有效率为97.1%,明显高于常规组的82.9%(χ^(2)=7.937,P=0.005);同时常规组患者中发生胃潴留、腹胀、腹泻和便秘的患者例数明显多于研究组(均P<0.05)。结论应用微生态免疫营养剂治疗脓毒症肠功能障碍患者的效果显著,不仅能减少患者血液中的炎症因子水平,还能提高患者的免疫力,改善患者肠道菌群,降低不良反应发生率,提高治疗总有效率,值得临床推广。Objective To explore the clinical effect of micro-ecological immune nutrition in sepsis patients with intestinal dysfunction and provide a reference for the treatment. Methods A total of 140 patients with sepsis and intestinal dysfunction in our hospital from March 2019 to February 2020 were randomly divided into the observation group or control Group, 70 patients in each group. Both groups received conventional nutritional treatment after admission. The patients in the observation group were additionally treated with micro-ecological immune nutrition for 14 days. The clinical effects were compared between groups at different time points. Results After treatment, the levels of CRP and PCT in both groups decreased significantly with those in observation group significantly lower than in control group(all P<0.05).Before treatment, there were no significant differences in the levels of CD3+ cell, CD4+cell, CD8+ cell, CD4+/CD8+, IgA, IgG, and IgM between the two groups. After 7 days and 14 days of treatment, the levels of these 7 indicators in the observation group were significantly higher than those in the control group(all P<0.05), respectively, and the amount of intestinal beneficial bacteria in observation group was significantly higher than in the control group(P<0.05). After treatment, the total efficiency rate in the observation group was 97.1%, significantly higher than in the control group(82.9%).The numbers of patients with gastric retention, bloating, diarrhea and constipation in the control group were significantly more than in the observation group(all P<0.05). Conclusion The combined use of micro-ecological immune nutrition has a significant clinical value in sepsis patients with intestinal dysfunction, which can effectively reduce the levels of indicators of inflammation, improve the immunity of patients, improve intestinal flora, and reduce the incidence of adverse reactions.
