右美托咪定在高血压脑出血术后临床应用的疗效观察

摘　　要：目的通过观察高血压脑出血患者术后相关指标的变化,探讨右美托咪定在高血压脑出血术后临床应用的疗效。方法选择我院神经外科2019年1月至2020年12月收治的高血压脑出血术后患者60例,随机分为试验组和对照组各30例。试验组静脉泵注右美托咪定,对照组静脉泵注咪达唑仑,记录并比较两组相关指标的变化。结果两组患者在治疗后HR、MAP呈下降趋势;试验组在T2、T3、T4的HR及MAP数值均低于对照组(P<0.05);试验组疼痛评分显著低于对照组(P<0.05);两组在镇静时间上相比较无显著差异(P>0.05);两组血糖值呈下降趋势,而Hs-CRP值呈上升趋势,在T1、T2、T3及T4时刻,试验组GLU值、Hs-CRP值均低于对照组(P<0.05)。结论右美托咪定可明显降低高血压脑出血术后患者的血压、心率,并能有效降低应激指标,且镇静、镇痛效果良好,临床疗效显著。Objective To investigate and analyze the clinical efficacy of dexmedetomidine in patients with hypertensive intracerebral hemorrhage by observing the changes of postoperative relevant indicators.Methods 60 postoperative patients with hypertensive intracerebral hemorrhage admitted to neurosurgery department of our hospital from January 2019 to December 2020 were randomly divided into the experimental group and the control group,30 patients in each group.The experimental group was treated with dexmedetomidine and the control group with midazolam.The changes of related indexes in the two groups were observed and compared.Results The HR and MAP of the two groups showed a downward trend after treatment.The HR and MAP level of the test group at T2,T3 and T4 were lower than those of the control group(P<0.05).The pain score of the test group was significantly lower than that of the control group(P<0.05).There was no significant difference in sedation time between the two groups(P>0.05).The blood glucose level of the two groups showed a downward trend,while the Hs-CRP level showed an upward trend.At T1,T2,T3 and T4,the GLU and Hs-CRP level of the test group were lower than those of the control group(P<0.05).Conclusion Dexmedetomidine can significantly reduce the blood pressure and heart rate of patients with hypertensive intracerebral hemorrhage after surgery,and can effectively reduce stress indicators with good sedative and analgesic effects.
