摘  要:目的分析CT靶扫描对不同类型肺结节的特征的显示能力。方法收集肺结节患者51例(57个结节),其中亚实性结节35个,实性结节22个,所有患者均行CT常规低剂量高分辨率扫描及靶扫描。分析不同扫描模式对两类结节的大小。内部有无钙化、空泡、血管集束征、支气管征及边缘有无分叶、毛刺、胸膜凹陷征显示能力。结果在实性结节中,高分辨率图像上测得实性结节的大小平均值为(12.35±2.45)mm,靶扫描组(13.48±3.91)mm,差异无统计学意义(P>0.05)。在结节内部有无钙化、空泡、血管集束征、支气管征及边缘有无分叶、毛刺、胸膜凹陷方面两者显示能力差异也无统计学意义(P>0.05)。亚实性结节中,在靶扫描图像上测得结节整体大小及部分实性结节的实性部分大小均大于高分辨率图像(P<0.05),不同分辨率对亚实性结节的毛刺征、血管集束征、支气管征的显示能力差异有统计学意义(P<0.05)。靶扫描模式的辐射剂量为(158.86±39.85),高于低剂量常规组和高分科率组的辐射剂量(104.29±27.21)。结论靶扫描超高分辨率重建对实性结节显示能力与低剂量高分辨率模式无差异,因此基于患者受益-辐射剂量最优化的原则,应仅对亚实性结节进行靶扫描,作为常规高分辨率扫描的必要补充,以提高其细节显示。Objective To amalyze the ability of CT target scan to display the characteristics of dfretr types of lung nodules.Methods A total of 51 patients with pulmonary nodules were ollected,a total of 57 nodules,including 35 sub-solid nodules and 22 solid nodules.All patients underwent conventional low-dose high-resolution CT scan and target scan.Analyze the ability of difrent scanming modes to show the size of the two types of nodules,the presence or absence of calcification,vacuoles,vascular bundling signs,bronchial signs,and the presence or absence of lobes,burs,and pleural depression signs.Results Among the solid nodules,the average size of the solid nodules measured on the high-resolution image was 12.35±2.45 tmm,the target scanning group(13.48±3.91 mm),but the dfferece between the two was not ststscally significant In addidion,there was no satistical diference in the ability to display calcifcation,vacuoles,vascular bundling,bronchial signs,lobes,burs,and pleural depresion in the nodules.In subsolid nodules,the overall size of the noduls and the size of the solid parts of some solid nodules measured on the caget scan image were larger than the high-resolution imges(P<0.05).There were dfferences in the ability to display the glitch sign.vascular bundle sign,and bronchial sign of solid nodules(P<0.05).The radiation dose(DLP)of the target scanning mode was 15886 t 39.85,which was higher than the radiaion dose(DLP)of the low-dose conventional group and the high-resolution group by 104.29±27.21.Conclusion The target scan ulta-high resolution reconstruction has mo difference in the display ability of solid nodules with the low-dose high-resolution mode.Therefore,based on the principle of patient benefit-opimization of radiation dose,only target scans should be performed on sub-solid nodules as Necessary addions to conventional high-resolution sc anning to improve the display of its detais.
