摘 要:目的:设计并探讨新型下肢康复步行训练器对脑卒中偏瘫患者康复训练的临床疗效。方法:辅助训练器可模拟脑卒中偏瘫患者日常行走锻炼,帮助患者进行室内外步行功能恢复训练。将此设备应用于我院2019-01~12收治的60例脑卒中偏瘫患者,其中男26例,女34例,年龄40~73岁,平均(59.24±5.54)岁。2018-01~12我院收治60例脑卒中偏瘫患者未使用下肢康复步行训练器的患者为对照组,其中男32例,女28例,年龄43~76岁,平均(59.24±5.54)岁。康复训练2、6、12个月后使用Fugl-Meyer法对下肢运动功能(FMA)评分、徒手肌力测试法(MMT)分级Kendall百分比法评分、Berg平衡功能量表(BBS)评分。结果:两组脑卒中偏瘫患者康复治疗前的FMA、MMT和BBS评分比较,差异无统计学意义(P>0.05);使用新型辅助训练器训练2、6、12个月后FMA、MMT和BBS评分均显著高于康复锻炼前,差异具有统计学意义(P<0.05或P<0.01);使用新型辅助训练器组患者的FMA、MMT和BBS评分显著高于未使用辅助训练器组(P<0.05或P<0.01)。结论:使用新型辅助训练器对脑卒中偏瘫患者进行康复锻炼后,患者肢运动功能、徒手肌力和平衡能力得到了明显的改善,通过本研究期望能为临床脑卒中偏瘫患者的临床康复提供帮助。Objective:To provide practical,safe and effective lower extremity rehabilitation equipment for stroke hemiplegic patients and other people with physical movement disorders.Methods:A new type of auxiliary training device was designed,including 10 structures such as panels,support columns,direction wheels,and seats.By simulating daily walking exercises,patients were assisted with indoor and outdoor walking function restoration training.At the same time,this device was applied to 60 stroke hemiplegia patients admitted to our hospital from January to December 2019,26 male and 34 female patients were in the age range of 40-73 years,the average age(59.24±5.54),and 60 stroke hemiplegia patients admitted from January to December 2018.The device was not used.The average age of the age range of 43 to 76 years(59.24±5.54)in 32 men and 28 women.Comparing the two groups of patients before and after rehabilitation exercise Fugl-Meyer lower extremity motor function(FMA)score,unarmed muscle strength test(MMT)rating Kendall percentage score,Berg balance function scale(BBS)score.Results:The new rehabilitation walking trainer was equipped with storage box and oxygen cylinder,the design was novel,the handbrake control layout was unique and easy to use,and the seat was equipped with safety protection device.Humanization of the overall design had a positive effect on improving the patient′s walking function.The FMA,Kendall and BBS scores of patients with hemiplegia before the rehabilitation of the two stroke groups were not statistically significant(P<0.05).The score of FMA,Kendall and BBS after 2 months of rehabilitation after using the new auxiliary training device was significantly higher than the difference before rehabilitation(P<0.05).The score of FMA,Kendall,and BBS in patients with new auxiliary trainers was significantly higher than that of those without auxiliary trainers(P<0.05).Conclusion:The rehabilitation of hemiplegic patients with cerebral apoplexy is improved by using a new auxiliary training device,and the motor
