摘 要:目的探究磁共振成像(MRI)联合电子计算机断层扫描(CT)诊断鼻咽癌早期颅底骨侵犯患者中的应用及优势。方法回顾性分析2018年1月至2020年1月滕州市中心人民医院肿瘤科收治的122例鼻咽癌患者的临床资料,男78例,女44例,年龄(65.77±4.23)岁,年龄范围为49~72岁。患者在治疗前均行CT检查及MRI检查,临床医师按照CT、MRI、CT联合MRI的检查结果进行临床决策,将定性诊断作为"金标准"。比较单独使用CT、单独使用MRI、CT联合MRI诊断鼻咽癌早期颅底骨侵犯与定性诊断的一致性,同时比较CT联合MRI诊断与分别单独使用CT或MRI诊断鼻咽癌早期颅底骨侵犯的灵敏度及特异度。结果CT诊断鼻咽癌早期颅底骨侵犯与CT联合MRI诊断的一致性比较,差异有统计学意义(k=0.874,P<0.05)。MRI诊断鼻咽癌早期颅底骨侵犯与CT联合MRI诊断一致性比较,差异有统计学意义(k=0.568,P<0.05)。CT诊断鼻咽癌早期颅底骨侵犯与CT联合MRI诊断的一致性明显高于MRI诊断鼻咽癌早期颅底骨侵犯与CT联合MRI诊断的一致性,差异有统计学意义(P<0.05)。CT联合MRI诊断鼻咽癌早期颅底骨侵犯的灵敏度及特异度均明显高于CT、MRI诊断,差异有统计学意义(P<0.05)。结论MRI联合CT诊断鼻咽癌早期颅底骨侵犯具有更加突出的灵敏度及特异度,早期诊断价值突出,能够为临床治疗方案提供可靠的影像学参考依据,可成为未来影像学研究的重点方向。Objective To explore and analyze the clinical application value of magnetic resonance imaging(MRI)combined with computed tomography(CT)in the diagnosis of early skull base invasion of nasopharyngeal carcinoma.Methods A retrospective analysis of the clinical data of 122 patients with nasopharyngeal carcinoma admitted to the Oncology of Tengzhou Central People′s Hospital from January 2018 to January 2020.There were 78 males and 44 females,with an age range of(65.77±4.23)years,49 to 72 years old.Patients undergo CT and MRI examinations before treatment.Clinicians make clinical decisions based on the results of CT,MRI,and CT combined with MRI,and regard qualitative diagnosis as the"gold standard."Compare the consistency of CT alone,MRI alone,CT combined with MRI in the diagnosis of early skull base invasion and qualitative diagnosis of nasopharyngeal carcinoma,and compare the diagnosis of CT combined with MRI and the use of CT or MRI alone in the diagnosis of early skull base invasion of nasopharyngeal carcinoma.The sensitivity and specificity.Results The consistency of CT diagnosis of early skull base invasion of nasopharyngeal carcinoma and CT combined with MRI diagnosis was statistically significant(k=0.874,P<0.05).The consistency of MRI diagnosis of early skull base invasion of nasopharyngeal carcinoma and CT combined with MRI diagnosis was statistically significant(k=0.568,P<0.05).The consistency of CT diagnosis of early skull base invasion of nasopharyngeal carcinoma and CT combined with MRI was significantly higher than that of MRI diagnosis of early skull base invasion of nasopharyngeal carcinoma and CT combined with MRI,and the difference was statistically significant(P<0.05).The sensitivity and specificity of CT combined with MRI in the diagnosis of early skull base invasion of nasopharyngeal carcinoma were significantly higher than those of CT and MRI,and the difference was statistically significant(P<0.05).Conclusion Compared with the diagnosis of early skull base bone invasion of nasopharyngeal carcin
