绷带型角膜接触镜合并玻璃酸钠在翼状胬肉切除合并角膜缘干细胞移植手术后的应用

摘　　要：目的研究绷带型角膜接触镜合并玻璃酸钠在翼状胬肉切除合并角膜缘干细胞移植手术后的临床应用效果。方法选取2019年7月至2020年12月在成都医学院第一附属医院眼科住院行翼状胬肉切除合并角膜缘干细胞移植手术的患者258例(共计258只眼)作为研究对象。按照随机数字表法分为A、B、C 3组,每组86例(86只眼)。A组术后单纯抗感染治疗,B组术后立即戴镜并抗感染治疗,C组术后立即戴镜并联合使用抗感染药物和玻璃酸钠治疗,观察术后1、2、5 d 3组患者视觉模拟评分(VAS)、角膜刺激症状、角膜上皮愈合情况。结果B、C组各时间点的VAS及角膜刺激症状均较A组低,差异有统计学意义(P<0.05),C组各时间点的VAS及角膜刺激症状均较B组低,差异有统计学意义(P<0.05);术后1、2 d B、C组角膜上皮愈合情况均较A组快,C组较B组快,差异均有统计学意义(P<0.05)。结论翼状胬肉切除合并角膜缘干细胞移植手术后联合使用绷带型角膜接触镜及玻璃酸钠,可有效减轻患者术后疼痛及角膜刺激症状,并可促进角膜上皮愈合。Objective To investigate the clinical effect of bandage contact lens(BCL)combined with sodium hyaluronate after pterygium excision combined with limbal stem cell transplantation.Methods A total of 258 eyes of 258 patients who underwent pterygium excision combined with limbal stem cell transplantation in the Department of Ophthalmology of the First Affiliated Hospital of Chengdu Medical College from July 2019 to December 2020 were selected and randomly divided into 3 groups with 86 cases(86 eyes)in each group.Immediately after surgery,group A was given anti-infective therapy alone,while group B was given BCL and anti-infective therapy,and group C was given BCL,anti-infective drugs and sodium hyaluronate.The visual analogue scale(VAS)score,corneal irritation symptoms and corneal epithelial healing were observed at 1,2 and 5 d after surgery.Results The VAS score and corneal irritation symptoms in group B and group C at each time point were lower than those in group A,and the differences were statistically significant(P<0.05).The VAS score and corneal irritation symptoms in group C at each time point were lower than those in group B,and the differences were statistically significant(P<0.05).The corneal epithelial healing at 1 and 2 d after surgery in group C was the fastest,folowed by group B and group A,and the difference was statistically significant(P<0.05).Conclusion After pterygium excision combined with limbal stem cell transplantation,combined use of BCL and sodium hyaluronate can effectively relieve postoperative pain and corneal irritation symptoms,and promote corneal epithelial healing,which is worthy of further promotion in clinical application.
