高血压脑出血患者继发吸入性肺炎影响因素分析

摘　　要：目的探讨高血压脑出血患者继发吸入性肺炎的影响因素。方法将102例高血压脑出血患者根据是否继发吸入性肺炎分为继发吸入性肺炎组30例,未继发吸入性肺炎组72例。调查两组患者一般资料,分析高血压脑出血患者继发吸入性肺炎的影响因素。结果继发吸入性肺炎组意识障碍、不良体位、吞咽障碍、胃食管反流、长期鼻饲患者比例显著高于未继发吸入性肺炎组(P<0.05或0.01)。意识障碍、不良体位、吞咽障碍、胃食管反流、长期鼻饲是高血压脑出血患者继发吸入性肺炎的影响因素(P<0.05或0.01)。结论意识障碍、不良体位、吞咽障碍、胃食管反流、长期鼻饲是高血压脑出血患者继发吸入性肺炎的影响因素,应引起临床重视,进行早期干预,降低患者继发吸入性肺炎风险。Objective To analyze the influencing factors for secondary aspiration pneumonia(AP)in patients with hypertensive cerebral hemorrhage(HCH).Methods The 102 patients with HCH were divided into secondary AP group(30 cases)and non-secondary AP group(72 cases)according to whether secondary aspiration pneumonia occurred.The general data of the two groups were investigated to analyze the influencing factors of secondary aspiration pneumonia in HCH patients.Results The proportion of patients with disturbance of consciousness,poor posture,dysphagia,gastroesophageal reflux and long-term nasal feeding in secondary AP group was significantly higher than that in non-secondary AP group(P<0.05 or 0.01).Disturbance of consciousness,poor posture,dysphagia,gastroesophageal reflux and long-term nasal feeding were the influencing factors of secondary aspiration pneumonia in HCH patients(P<0.05 or 0.01).Conclusions Disturbance of consciousness,poor posture,dysphagia,gastroesophageal reflux,and long-term nasal feeding are the influencing factors of secondary AP in HCH patients.Clinical attention should be paid to early intervention to reduce the risk of secondary aspiration pneumonia in patients.
