长期住院精神分裂症患者抑郁情绪与人格特征、孤独感的相关性分析

摘　　要：目的探讨长期住院精神分裂症患者抑郁情绪与人格特征、孤独感的相关性。方法选取109例长期住院精神分裂症患者为研究组,选取社区181名居民为对照组;根据汉密尔顿抑郁量表(HAMD-17项)评分将研究组患者分为轻度抑郁组(43例)、中度抑郁组(54例)及重度抑郁组(12例);采用一般情况调查表、艾森克人格问卷简式量表中国版(EPQ-RSC)及UCLA孤独感量表对入组者进行问卷调查,分析长期住院精神分裂症患者抑郁情绪与人格特征、孤独感的相关性。结果研究组HAMD、UCLA总分及EPQ-RSC精神质、神经质评分均高于对照组,EPQ-RSC外倾性评分低于对照组(P<0.01)。研究组轻度抑郁组、中度抑郁组、重度抑郁组UCLA总分及EPQ-RSC外倾性、神经质维度评分比较差异均有统计学意义(P<0.01)。进一步两两比较结果显示,中度抑郁组EPQ-RSC外倾性评分低于轻度抑郁组(P<0.05),重度抑郁组EPQ-RSC神经质评分高于轻度抑郁组(P<0.05),中度抑郁组和重度抑郁组UCLA总分均高于轻度抑郁组(P<0.05)。相关分析显示,研究组HAMD与EPQ-RSC外倾性呈负相关(P<0.05),与EPQ-RSC神经质、UCLA均呈正相关(P<0.05)。结论长期住院精神分裂症患者存在抑郁情绪,且与部分人格特征及孤独感有关。Objective To explore the correlations between depression and personality characteristics,loneliness in long-term inpatients with schizophrenia.Methods A total of 109 long-term inpatients with schizophrenia in study group,and 181 community residents in control group were selected and the study group was divided into mild depression group(43 cases),moderate depression group(54 cases)and severe depression group(12 cases)according to the scores of Hamilton Depression Scale(HAMD-17).The general information questionnaire,Eysenck Personality Questionnaire Chinese Version(EPQ-RSC)and UCLA Loneliness Scale were used to analyze the correlations between depression and personality characteristics,loneliness in long-term inpatients with schizophrenia.Results The total scores of HAMD,UCLA and factor scores of psychoticism,neuroticism of EPQ-RSC in study group were all significantly higher than those in control group,and the extraversion factor score of EPQ-RSC was significantly lower than that in control group(P<0.01).There existed significant differences in the total scores of UCLA and factor scores of extraversion,neuroticism of EPQ-RSC among mild depression group,moderate depression group and severe depression group(P<0.01).Compared with mild depression group,moderate depression group had lower extraversion factor score of EPQ-RSC and higher total score of UCLA(P<0.05),while severe depression group had higher neuroticism factor score of EPQ-RSC and higher total score of UCLA(P<0.05).Pearson correlation analysis showed that the total score of HAMD was negatively correlated with extraversion of EPQ-RSC(P<0.05),and positively correlated with neuroticism of EPQ-RSC as well as the total score of UCLA(P<0.05).Conclusion Long-term inpatients with schizophrenia have depression,which is related to some personality characteristics and loneliness.
