拮抗电针法联合康复训练、空气波压力循环治疗仪治疗卒中后痉挛性瘫痪疗效观察

摘　　要：目的观察拮抗电针法联合康复训练、空气波压力循环治疗仪治疗卒中后痉挛性瘫痪的临床疗效。方法将110例卒中后痉挛性瘫痪患者按照随机数字表法分为2组。对照组55例予康复训练、空气波压力循环治疗仪治疗,治疗组55例在对照组治疗基础上联合拮抗电针法治疗。2组疗程为6周。比较2组治疗前后痉挛程度、Fugl-Meyer运动功能评估量表评分、肌活动能力及日常生活活动能力(ADL)量表评分,并统计疗效。结果治疗组总有效率96.4%(53/55),对照组总有效率78.2%(43/55),治疗组疗效优于对照组(P<0.05)。治疗后2组痉挛程度均较本组治疗前改善(P<0.05),且治疗后治疗组痉挛程度改善情况优于对照组(P<0.05)。治疗后2组上、下肢Fugl-Meyer运动功能评分均较本组治疗前升高(P<0.05),且治疗后治疗组均高于对照组(P<0.05)。治疗后2组腓肠肌、胫骨前肌电位值及屈髋肌力、伸肌肌力徒手肌力评定量表(MMT)评分均升高(P<0.05),下肢肌力评分均降低(P<0.05),且治疗后治疗组腓肠肌、胫骨前肌电位值及屈髋肌力、伸肌肌力MMT评分均高于对照组(P<0.05),下肢肌力评分低于对照组(P<0.05)。治疗后2组ADL评分均较本组治疗前升高(P<0.05),且治疗后治疗组高于对照组(P<0.05)。结论拮抗电针法联合康复训练、空气波压力循环治疗仪可有效治疗卒中后痉挛性瘫痪,改善痉挛程度、肢体功能和肌活动能力,提高卒中患者的康复效果。Objective To observe the curative effect of antagonistic electroacupuncture(AEA)combined with rehabilitation training and air pressure wave cycle therapy apparatus on spastic paralysis after stroke.Methods Totally 110 patients with spastic paralysis after stroke were randomly divided into two groups,55 cases in control group were treated with rehabilitation training,and air wave pressure circulation therapy device,and 55 cases in treatment group were given AEA based on control group,treated for 6 weeks.The key observation was the degree of muscle spasm,Fugl-Meyer assessment for motor recovery,muscle activity,and activities of daily living(ADL)scale score,and efficacy was counted.Results The total effective rate was better in treatment group than in control group[96.4%(53/55)vs 78.2%(43/55),(P<0.05),respectively].Compared with before treatment,the degree of muscle spasm in groups was improved after treatment(P<0.05),which was better in treatment group than control group(P<0.05);the upper and lower limbs Fugl-Meyer for motor function scores in groups were increased after treatment(P<0.05),which in treatment group were significantly increased(P<0.05);manual muscle test(MMT)scores(potential values of gastrocnemius and tibial anterior muscle,muscle strength of tibial anteriorand hip flexor and extensor)were increased after treatment(P<0.05),which in treatment group were significantly increased(P<0.05);and the lower limb muscle strength scores were decreased(P<0.05),which in treatment group were significantly decreased(P<0.05);ADL scores were higher after treatment in groups(P<0.05),and which in treatment group was significantly increased(P<0.05).Conclusion For patients with spastic paralysis after stroke,AEA combined with rehabilitation training and air wave pressure circulation therapy device can effectively treat spastic paralysis after stroke,improve the spasm degree,limb function and muscle activity,and enhance the rehabilitation effect.
