摘 要:目的探究CYP7A1基因多态性与乙型肝炎病毒感染严重程度的相关性。方法选取2018年1月-2020年1月浙江省湖州市中心医院收治的213例乙型肝炎病毒感染患者为感染组,并根据病毒感染损伤肝功能的严重程度(Child-Pugh)分为A组(A级68例)、B组(B级49例)、C组(C级96例)。选取同期于医院行健康体检无肝病的健康志愿者80名为对照组。采集血液样本,进行CYP7A1基因多态性分析。结果感染组与对照组rs3824260位点CT、CC基因型、rs4738687位点GA、GG基因型、rs8192871位点CT、TT基因型频率比较差异具有统计学意义(P<0.05)。感染组rs3824260位点C等位基因、rs4738687位点G等位基因、rs8192871位点T等位基因携带率高于对照组(P<0.05)。乙型肝炎病毒感染患者不同感染程度rs3824260位点CT、CC基因型、rs4738687位点GA、GG基因型、rs8192871位点CT、TT基因型频率比较差异具有统计学意义(P<0.05),rs3824260位点携带等位基因C、rs4738687位点携带等位基因G、rs8192871位点携带等位基因T与感染组感染严重程度相关。结论CYP7A1基因rs3824260、rs4738687、rs8192871位点多态性为乙型肝炎病毒感染的易感基因,C等位基因、G等位基因、T等位基因是增加感染程度的危险基因,临床可根据其预测评估此病。OBJECTIVE To explore the association between CYP7A1gene polymorphism and severity of hepatitis B virus(HBV)infection.METHODS A total of 213patients with HBV infection who were treated in Huzhou Central Hospital of Zhejiang Province from Jan 2018to Jan 2020were assigned as the infection group and were divided into the group A with 68cases,the group B with 49cases,and the group C with 96cases according to the severity of liver function damage(Child-Pugh),meanwhile,80healthy people who received physical examination were chosen as the control group.Blood specimens were collected.RESULTS There were significant differences in the frequencies of CT,CC genotypes at rs3824260locus,GA,GG genotypes at rs4738687locus,and CT,TT genotypes at rs8192871locus between the infection group and the control group(P<0.05).The carrying rates of C allele at rs3824260locus,G allele at rs4738687locus and T allele at rs8192871locus were significatly higher in the infection group than in the control group(P<0.05).There were significant differences in the frequencies of CT,CC genotypes at rs3824260locus,GA,GG genotypes at rs4738687locus,and CT,TT genotypes at rs8192871locus among the patients with different degree of hepatitis B virus infection(P<0.05).The C allele at rs3824260locus,G allele at rs4738687locus and T allele at rs8192871locus were associated with the severity of infection in the infection group.CONCLUSIONThe CYP7A1gene morphisms at rs3824260,rs4738687and rs8192871loci are the susceptibility genes for the hepatitis B virus infection.C allele,G allele and T allele are the risk genes to increase the degree of infection,the clinicians can assess the diseases based on those indexes.
