摘　　要：目的探讨乙型肝炎病毒(HBV)感染孕妇外周血微小核糖核酸-20a(miR-20a)、miR-1296、miR-153-1表达与HBV病毒载量的关系。方法回顾性选取2017年1月-2020年1月于郑州大学第一附属医院行产前检查的113例HBV感染孕妇作为研究对象,设为感染组;另收集同期于医院产检的无HBV感染孕妇80名设为非感染组,以及于医院进行体检的健康女性40名作为对照并设为对照组,检测和比较三组外周血miR-20a、miR-1296、miR-153-1表达水平、HBV-DNA载量,比较感染组不同HBV-DNA载量孕妇的血清白细胞介素-16(IL-16)、IL-18、单核细胞趋化蛋白-1(MCP-1)水平,分析miR-20a、miR-1296、miR-153-1与HBV-DNA载量、血清免疫细胞因子IL-6、IL-8、MCP-1的关系;并对感染组孕妇进行1年随访,比较不同HBV-DNA载量孕妇的分娩结局。结果三组miR-20a、miR-1296、miR-153-1相对表达量差异有统计学意义(P<0.05),其中,感染组外周血miR-20a、miR-1296、miR-153-1表达量均高于非感染组和对照组(P<0.05)。113例HBV感染孕妇中HBVDNA载量<103 copies/ml 71例,HBV-DNA载量≥103 copies/ml 42例。HBV-DNA阳性组外周血miR-20a、miR-1296、miR-153-1表达量和血清IL-16、IL-18、MCP-1水平均高于HBV-DNA阴性组(P<0.05);相关性分析结果显示,HBV感染孕妇miR-20a、miR-1296、miR-153-1表达分别与HBV-DNA载量、IL-16、IL-18和MCP-1呈正相关(P<0.05)。随访后,HBV-DNA阳性组孕妇娩出新生儿窒息和HBV感染发生率均高于阴性组(P<0.05)。结论miR-20a、miR-1296、miR-153-1在HBV感染孕妇外周血中呈较高表达,并与HBV-DNA载量、IL-16、IL-18、MCP-1呈正相关。OBJECTIVE To explore the relationship between the expression of microRNA-20a(miR-20a),microRNA-1296(miR-1296),microRNA-153-1(miR-153-1)and hepatitis B virus(HBV)load in the peripheral blood of pregnant women infected with HBV.METHODS A total of 113pregnant women with HBV infection undergoing prenatal check-ups in the First Affiliated Hospital of Zhengzhou University from January 2017to January 2020 were retrospectively selected as the study subjects,and set them as the infection group.In addition,80pregnant women without HBV infection who underwent prenatal check-up in the hospital during the same period were set as non-infection group,and 40healthy women who underwent physical examination in the hospital were enrolled as control group.The expression levels of miR-20a,miR-1296and miR-153-1,and HBV-DNA load in the peripheral blood of the three groups were detected and compared.Serum levels of interleukin-16(IL-16),interleukin-18(IL-18)and monocyte chemoattractant protein-1(MCP-1)in pregnant women with different HBV-DNA loads in infection group were compared.The relationship between miR-20a,miR-1296,miR-153-1and HBV-DNA load,serum immune cytokines(IL-6,IL-8,MCP-1)was analyzed.The pregnant women in infection group were followed up for 1year to compare the delivery outcomes of pregnant women with different HBV-DNA loads.RESULTS There were significant differences in the relative expression levels of miR-20a,miR-1296and miR-153-1among the three groups(P<0.05).Among them,the expression levels of peripheral blood miR-20a,miR-1296and miR-153-1in infection group were significatly higher than those in non-infection group and control group(P<0.05).Among the 113pregnant women with HBV infection,there were 71cases with HBV-DNA load lower than 103 copies/ml and 42cases with HBV-DNA load not lower than 103 copies/ml.The expression levels of peripheral blood miR-20a,miR-1296and miR-153-1,and levels of serum IL-16,IL-18and MCP-1in HBV-DNA positive group were significatly higher than those in HBV-DNA negative group(P<0.05).The resu
