正畸联合修复体修复关闭牙列散在间隙对咬合功能及牙龈乳头美学的影响

摘　　要：目的:探讨正畸联合修复体修复关闭牙列散在间隙对牙列功能的改善作用及其对牙龈乳头美学的影响。方法:选择42例牙列散在间隙患者,应用正畸联合修复体修复关闭牙列间隙,评价整体治疗效果,比较治疗前后咬合功能、咀嚼功能及牙龈乳头美学变化。结果:42例患者均成功关闭牙列间隙,临床有效率为92.9%,患者治疗后颌力总值升高(P<0.05)、颌力不对称指数下降(P<0.05)、颌力中心点最大位移减少(P<0.05),治疗后吞咽阈值咀嚼时间、吞咽阈值咀嚼次数均较治疗前降低(P<0.05),咀嚼硬性食物及咀嚼软性食物评分均较治疗前提高(P<0.05)。治疗后牙龈出血指数、探诊指数及牙龈指数较前降低(P<0.05),齿龈乳头指数较前增高(P<0.05)。治疗后牙龈美学满意度评分较前提高(P<0.05)。结论:正畸联合修复体修复关闭牙列散在间隙能够改善牙列外观及咬合功能,提高牙周软组织情况,改善牙龈乳头美学效果。Objective To investigate the effect of orthodontic closure combined with prosthesis on the improvement of dentition function and aesthetics of gingival papillae.Methods Forty-two patients with dispersed teeth were treated with individualized orthodontic treatment.The overall treatment effect was evaluated.The occlusal function,masticatory function and aesthetic changes of gingival papilla were compared before and after treatment.Results 42 patients were successful in orthodontic restoration of teeth scattered in the gap,the clinical excellent rate was 92.9%.After treatment,the total force of patients increased(P<0.05),force asymmetry index decreased(P<0.05),force center maximum displacement decreased(P<0.05).After treatment,the swallowing threshold mastication time and the number of swallowing threshold mastication times were all lower than those after treatment.The scores of chewing hard food and soft food were higher than those before treatment(P<0.05).After treatment,gingival bleeding index,probe index and gingival index decreased(P<0.05)and gingival papilla index increased(P<0.05).After treatment,the gingival aesthetic satisfaction score was higher than that before treatment(P<0.05).Conclusion Individualized orthodontic closure combined with prosthesis can improve the appearance of dentition and occlusal function,improve the treatment of periodontal soft tissue,and improve the aesthetic effect of gingival papilla.
